摘 要:目的研究定量MRI与膝关节负重区早期退变软骨细胞外基质成分改变的相关性。方法取15例行全膝关节置换术的骨关节炎患者截骨后的外侧股骨髁和外侧胫骨平台负重区软骨标本,分别利用7.0 T磁共振T2mapping和d GEMRIC技术体外测量膝关节负重区早期退变软骨的T2值和ΔR1值,计算同部位软骨糖胺聚糖(GAG)含量。分析T2值和ΔR1值与GAG含量的相关性。结果 15例标本负重区软骨的T2值为24.4~38.7 (28.7±3.2) ms,ΔR1值为1.40~1.92 (1.62±0.50) ms,GAG含量为39.6~55.3 (44.8±2.6)mg/g。T2值与GAG含量无明显相关(r=-0.040,P=0.200);ΔR1值与GAG含量呈负相关(r=-0.430,P=0.001)。结论人膝关节负重区早期退变软骨的T2值与细胞外基质成分中GAG含量无明显相关,而ΔR1值与GAG含量呈负相关。MRI d GEMRIC技术能够在无创条件下用于诊断早期膝关节骨关节炎及监测疾病治疗效果。Objective To investigate the correlation of quantitative magnetic resonance imaging and knee early stage osteoarthritis of weight-bearing region of degraded cartilage extracellular matrix biochemical composition.Methods Cartilage specimens of lateral femoral condyle and lateral tibial plateau were collected from 15 osteoarthritis patients undergoing total knee arthroplasty.7.0 T magnetic resonance T2 mappingand d GEMRIC imaging technique were used to measure T2 values and ΔR1 values respectively in weight-beating area,calculate the glycosaminoglycan( GAG)content in the same area of cartilage.The correlation between T2 value and ΔR1 value and GAG content was analyzed respectively.Results The T2 value of the load-bearing cartilage in 15 patients was 24.4 ~ 38.7( 28.7 ± 3.2) ms and the value ΔR1 was 1.40 ~ 1.92( 1.62 ± 0.50) ms.The value of GAG content was 39.6 ~ 55.3( 44.8 ± 2.6)mg/g.T2 values were no correlation with GAG concentration( r =-0.040,P = 0.200);ΔR1 values were inversely correlated with GAG concentration( r =-0.430,P = 0.001).Conclusions The T2 values are no correlationed with GAG content of human knee osteoarthritis of weight-beating region degraded cartilage.ΔR1 values are inversely correlated with GAG concentration.d GEMRIC serve as a non-invasive imaging technique can be used to diagnose early stage knee osteoarthritisand monitor the treatment efficacy.
