摘　　要：目的探讨中西药联用对CO2激光显微手术后喉癌前病变患者喉功能的改善作用。方法选取浙江省桐乡市第一人民医院和杭州师范大学附属医院耳鼻咽喉科收治的喉癌前病变患者88例进行研究,用随机数字表法分为两组。对照组44例,以CO2激光显微手术联合术后常规抗生素治疗;观察组44例,CO2激光显微手术后行布地奈德雾化吸入联合自拟中药汤剂治疗,对比两组治疗效果。结果术前,两组患者声带形态学评分、嗓音声学各指标比较差异无统计学意义(P>0.05);术后4周,观察组患者谐噪比、基频、标准化噪声能量、振幅微扰均明显优于对照组(P<0.05)。两组患者远期疗效比较,观察组术后1年复发率(4.6%)及恶变率(0%)均明显低于对照组(P<0.05)。结论布地奈德雾化吸入联合自拟中药汤剂可改善CO2激光显微手术后喉癌前病变患者喉功能。Objective To investigate the improvement of traditional Chinese medicine and western medicine combination on the laryngeal function of patients with laryngeal precancerous lesions after CO2 laser microsurgery.Methods A total of 88 patients with laryngeal precancerous lesions admitted to Department of Otorhinolaryngology,the Affiliated Hospital of Hangzhou Normal University and Tongxiang First People′s Hospital of Zhejiang from January 2015 to August 2017 were selected for the study and divided into 2 groups based on random number table.The control group consisted of 44 patients treated with CO2 laser microsurgery combined with postoperative routine antibiotics,and the observation group consisted of 44 patients treated with Budesonide aerosol inhalation and self-made traditional Chinese medicine decoction after CO2 laser microsurgery.The therapeutic effects of the two groups were compared.Results Before operation,there was no significant difference between the two groups of patients in morphological score of vocal cords and indexes of voice acoustics ( P >0.05).After 4 weeks,patients in the observation group were significantly better than those in the control group in terms of harmonic noise ratio,fundamental frequency,standardized noise energy and amplitude perturbation( P <0.05).In the comparison of long-term efficacy of the two groups,the postoperative 1-year recurrence rate (4.6%) and malignant rate (0%) of the observation group were significantly lower than those of the control group ( P <0.05).Conclusion Budesonide aerosol inhalation combined with self-made traditional Chinese medicine decoction has a improvement on the laryngeal function of patients with laryngeal precancerous lesions after CO2 laser microsurgery.
