血清转铁蛋白受体、铁蛋白对ACD与IDA的诊断价值

摘　　要：目的:探讨血清转铁蛋白受体、铁蛋白对慢性病贫血(chronic disease anemia,ACD)与缺铁性贫血(iron deficiency anemia,IDA)的鉴别诊断价值。方法:选择123例ACD或IDA患者,分为IDA组(61例)及ACD组(62例);选择同期51例健康体检者为对照组;检测三组患者的血清转铁蛋白受体(serum transferrin receptor,sTfR)、血清铁蛋白(serum ferritin,SF)水平,对比受试者的sTfR、SF水平,sTfR及SF指标下的ROC曲线面积、敏感度及特异度。结果:三组间的sTfR、SF对比差异具有统计学意义( P <0.05);sTfR水平:IDA组>ACD组>对照组;SF水平:ACD组>对照组>IDA组;sTfR的ROC曲线下面积及敏感度高于SF( P <0.05)。结论:sTfR对ACD与IDA的鉴别诊断价值优于SF。Objective:To explore the differential diagnostic value of serum transferrin receptor and ferritin in chronic disease anemia (ACD) and iron deficiency anemia (IDA). Methods:123 patients with ACD or IDA admitted to our hospital from January 2015 to June 2018 were divided into IDA group (61 cases) and ACD group (62 cases) according to the diagnostic criteria of IDA and ACD. At the same time, 51 healthy persons were selected as control group. Serum transferrin receptor (sTfR) and serum ferritin (SF) levels were measured in three groups. The levels of sTfR and SF, the area of ROC curve under sTfR and SF, sensitivity and specificity were compared. Results:There were significant differences in sTfR and SF among the three groups ( P <0.05). STfR level: IDA group > ACD group > control group;SF level: ACD group > control group > IDA group. The area of ROC curve and sensitivity of sTfR was significantly higher than that of SF, P <0.05. There was no significant difference between the specificity of sTfR and SF, P >0.05. Conclusion:STfR is superior to SF in the differential diagnosis of ACD and IDA, and is worthy of clinical application.
