摘 要:目的 探讨白细胞介素-6受体(IL-6R)、白细胞介素-27(IL-27)基因多态性与冠心病易感性的关系。方法 选择本院心血管内科2015年6月—2017年8月收治的冠心病病人226例为试验组,另选择200名身体健康者为对照组。收集所有受试者血浆总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、凝血四项指标[包括凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)]等。利用高分辨率溶解曲线(PCR-HRM)技术分析IL-6R和IL-27基因多态性与冠心病易感性的相关性。结果 ①IL-6R(183A/G)检测到3种基因型:AA、AG、GG;IL-6R(exon1A/C)检测到3种基因型:AA、AC、CC;IL-6R(exon2A/T)检测到2种基因型:AT、TT。IL-6R(183A/G)的基因型及等位基因频率在试验组和对照组间比较差异无统计学意义(P>0.05),而IL-6R(exon1A/C)和IL-6R(exon2A/T)在试验组和对照组间差异均有统计学意义(P<0.05)。②IL-27(4730T/C)检测到2种基因型:TT、TC;IL-27(-964A/G)检测到3种基因型:AA、AG、GG;IL-27(2905T/G)检测到2种基因型:TT、TG。IL-27(4730T/C)、IL-27(-964A/G)和IL-27(2905T/G)的基因型及等位基因频率在试验组和对照组间的差异均无统计学意义(P>0.05)。③IL-6R(exon1A/C)的突变纯和基因型CC与野生纯合型AA间的差异有统计学意义(P<0.05),IL-6R(exon2A/T)的突变杂合基因型AT与野生纯合型TT间的差异有统计学意义(P<0.05)。④IL-6R(exon1A/C)的基因型与血脂指标TC、HDL-C显著相关(P<0.05)。结论 IL-6R(exon1A/C)、IL-6R(exon2A/T)多态性与冠心病易感性有关,而IL-27基因多态性与冠心病易感性无关。Objective To investigate the relationship between the polymorphism of interleukin-6 receptor (IL-6R) and interleukin-27 (IL-27) gene and the susceptibility to coronary heart disease (CHD). Methods Two hundred and twenty-six patients with CHD in our Cardiovascular Department from June2015 to August2017 were selected as the experimental group,and200 healthy individuals were selected as control group.The plasma total cholesterol (TC),glycerin three greases (TG),low density lipoprotein cholesterol (LDL-C),high density lipoprotein cholesterol (HDL-C) and blood coagulation four indices including prothrombin time (PT),activated partial thromboplastin time (APTT),thrombin time (TT),and fibrinogen (FIB) were collected.The correlation between IL-6R and IL-27 gene polymorphisms and susceptibility to coronary heart disease was analyzed by polymerase chain reaction (PCR) high resolution melting (PCR-HRM) curve technique. Results AA,AG,and GG genotypes were detected in IL-6R (183A/G).AA,AC and CC genotypes were detected in IL-6R (exon1A/C).AT and TT genotypes were detected in IL-6R (exon2A/T).There was no significant difference in the genotype and allele frequencies of IL-6R (183A/G) between the experimental group and the control group ( P >0.05).There were differences in IL-6R (exon1A/C) and IL-6R (exon2A/T) between the experimental group and the control group ( P <0.05).TT and TC genotypes were detected in IL-27 (4730T/C).AA,AG,and GG genotypes were detected in IL-27 (-964A/G).TT and TG genotypes were detected in IL-27 (2905T/G).There was no significant difference in genotype and allele frequencies of IL-27 (4730T/C),IL-27 (-964A/G) and IL-27 (2905T/G) between the experimental group and the control group ( P >0.05).There were differences between the mutant pure,the genotypic CC and the wild homozygous AA of the IL-6R (exon1A/C)( P <0.05).There was significant difference between the mutant heterozygous genotype AT and wild homozygous TT of the IL-6R (exon2A/T)( P <0.05).The genotype of IL-6R (exon1A/C) was significantly corre
