早期微量喂养深度水解蛋白配方奶对极低出生体质量儿生长发育的影响

摘　　要：目的 探讨深度水解蛋白配方奶在极低出生体质量儿(VLBWI)早期喂养中的效果,为VLBWI的早期胃肠营养提供科学依据。方法 选取2018年1—12月山东省立医院新生儿科收治的176例VLBWI作为研究对象,按实际喂养方式分为观察组72例和对照组104例。观察组早期给予深度水解蛋白配方奶微量喂养,逐步达到每次5ml喂养量,耐受后改用等量的标准早产儿配方奶继续喂养;对照组给予标准早产儿配方奶喂养。比较两组VLBWI生长发育情况。结果 观察组患儿恢复出生体质量日龄较对照组短(P<0.05),住院时间短于对照组(P<0.05),出院体质量高于对照组(P<0.05);宫外生长发育迟缓率低于对照组,但无统计学意义(P>0.05)。结论 深度水解蛋白配方奶早期应用在促进VLBWI生长发育方面有一定的优势,值得在临床推广。Object To explore the effect of deep hydrolyzed protein formula milk powder in the early feeding of very low birth weight infants (VLBWI), thus providing the scientific basis for early gastrointestinal nutrition of VLBWI. Methods We selected 176 cases of VLBWI who were admitted to the department of neonatology, Shandong Provincial Hospital from January 2018 to December 2018 as the study subjects. According to the actual feeding methods, 72 cases were set as the observation group and 104 cases were set as the control group. In the observation group, micro-feeding of deep hydrolyzed protein formula milk powder was given at the early stage, and the amount of feeding was gradually increased to 5 ml each time. After the children tolerated the powder, we gave them standard preterm infant formula milk powder for further feeding. The control group was given standard preterm infant formula milk powder feeding. The growth and development of VLBWI in the two groups were compared. Results In the observation group, the dates for recovery of birth body mass was shorter than that of the control group( P <0.05), the hospitalization time was shorter than that of the control group( P <0.05), and the discharge body mass was higher than that of the control group( P <0.05). The rate of extrauterine growth retardation was lower than that of the control group, but the comparative difference was not statistically significant( P >0.05). Conclusion Early application of deep hydrolyzed protein formula milk powder has certain advantages in promoting the growth and development of VLBWI,which was worthy of clinical promotion.
