川芎茶调散和盐酸氟桂嗪胶囊治疗偏头痛的效果对比

摘　　要：目的:对比评价川芎茶调散和盐酸氟桂嗪胶囊治疗偏头痛的临床效果,并探讨其可能存在的作用机制。方法:选取2017年1月-2018年1月至我院中医科门诊治疗的偏头痛患者90例,按随机数表法分为观察组和对照组,每组各45例,分别采用川芎茶调散和盐酸氟桂嗪胶囊治疗。对比两组患者治疗效果,视觉模拟评分法(Visual Analogue Scale,VAS)评分、五羟色胺(5-HT)水平、β-内啡肽水平改变以及不良反应发生率。结果:观察组治疗效果显著优于对照组,差异有统计学意义(P<0.05);治疗前,两组患者VAS评分对比,差异无统计学意义(P>0.05);治疗后,VAS显著下降且观察组低于对照组,差异有统计学意义(P>0.05);治疗1疗程后,对照组VAS评分与治疗前对比,差异无统计学意义(P>0.05);治疗2个疗程后对照组VAS评分显著低于治疗前,差异有统计学意义(P<0.05);治疗前两组患者5-HT以及β-内啡肽对比,差异无统计学意义(P>0.05);治疗后,观察组5-HT以及β-内啡肽显著升高,差异有统计学意义(P<0.05);治疗后,对照组5-HT以及β-内啡肽与治疗前对比,差异无统计学意义(P>0.05);观察组患者不良反应显著低于对照组,差异有统计学意义(P<0.05)。结论:采用川芎茶调散治疗偏头痛,具有较好的有效性和安全性,镇痛作用明显,可能与调节5-HT以及β-内啡肽水平相关。Objective: To compare the clinical effects of Chuanxiong Chatiao San (川芎茶调散) and flunarizine hydrochloride capsules on migraine, and explore its possible mechanism. Methods: 90 migraine patients were divided into the observation group and the control group, 45 cases in each. The observation group was given Chuanxiong Chatiao San;the control group was treated with flunarizine hydrochloride capsules. Results: The efficacy in the observation group was significantly better than the control group (P<0.05). After treatment, the VAS scores were decreased significantly in groups;those in the observation group was lower than the control group (P>0.05). After 1 course, there was no statistically significant difference of VAS score in the control group compare with before treatment;after 2 courses, the VAS score in the control group was lower than that before treatment. After treatment, 5-HT and β-endorphins in the observation group were significantly increased (P<0.05);there was no significant difference compared with before treatment in the control group (P>0.05). The adverse reactions in the observation group were significantly lower than the control group, with a statistically significant difference (P<0.05). Conclusion: Chuanxiong Chatiao San on migraine shows good effects and safety, can obviously relieve analgesia, which may be related to the regulation of 5-HT and β-endorphin levels.
