单臂外固定支架固定治疗胫腓骨中下段骨折70例

摘　　要：目的:观察单臂外固定支架固定治疗胫腓骨中下1/3段骨折的疗效。方法:在腰麻或连硬外麻醉下,开放性骨折彻底清创,闭合性骨折先行手法复位,骨折复位满意后,在C型臂电视X光机监视下安装单臂外固定支架固定,治疗胫腓骨中下1/3段骨折70例,随访6个月~3年,平均16个月。结果:优42例,良23例,可5例,差0例。结论:单臂外固定支架治疗胫骨腓中下1/3段骨折能够达到牢固的三维固定,避免发生旋转畸形,恢复胫腓骨的正常力线,符合骨折愈合的生物力学环境。该治疗方法具有创伤小、骨折愈合率高、拆除外固定支架无需麻醉及费用低廉的优点,值得在临床广泛使用。Objective:To observe the effect of unilateral external fixation on middle and distal 1/3 fractures of tibia and fibula. Methods:Under lumbar anesthesia or epidural anesthesia,open fracture was thoroughly debrided;closed fracture was treated by manual reduction.After satisfactory reduction,single arm external fixator was installed under the supervision of C-arm TV X-ray machine.Followup period ranged from six months to three years,with an average of 16 months.Results:In 70 cases,42 cases were excellent;23 cases were good;five cases was fair and 0 case was poor.Concusion:Unilateral external fixation on middle and distal 1/3 fractures of tibia and fibula can achieve solid three-dimensional fixation,avoid rotation deformity and restore the normal force line of tibia,which conforms to the biomechanical environment of fracture healing,shows the advantages of small trauma,high fracture healing rate,no need of anesthesia and low cost,and is worthy of wide clinical use.
