糖尿病足患者抑郁发生的影响因素及中医证型研究

摘　　要：目的:分析糖尿病性足病(diabetic foot,DF)患者发生抑郁的影响因素。方法:选取DF患者105例,采集患者一般资料及病史资料,使用汉密尔顿抑郁量表(HAMD-24)、中医证候观察表进行调查。结果:DF患者抑郁发生率高于正常人群,单因素分析显示Wagner分级高、糖化血红蛋白升高、有经济困难、有截肢/趾史或风险、有手术/介入史或预期的DF患者发生抑郁的比例显著增高(P<0.05);Logistic回归分析提示经济困难(OR=3.267)、有手术史或风险(OR=16.574)、Wanger分级(OR=4.913)为DF患者发生抑郁的危险因素;中医证型为气滞血瘀、湿热困脾的患者抑郁患病率较高。结论:经济困难、有手术史或风险、Wagner分级高可能是DF患者发生抑郁的危险因素,中医证型为气滞血瘀、湿热困脾的患者抑郁患病率较高。Objective:To analyze the related affecting factors and TCM syndromes of patients with Diabetic Foot (DF).Methods: Totally 105 patients diagnosed as DF were selected.General information,medical history data and TCM information of the patients were collected,HAMD questionnaire was used to investigate.Results:The rate of depression was higher in patients with DF than in the national norm.The degree of DF (Wagner),economic difficulty,history or risk of amputation,history or expectation of surgery or interventional therapy showed significant difference of depression in patients with DF (P<0.05).The logistic regression analysis showed that economic difficulty(OR = 3.267),history or expectation of surgery or interventional therapy (OR = 16.574),the degree of Wanger(OR = 4.913) were risk factor of depression in patients with DF.Prevalence of depression was higher in syndrome of dampness-heat and syndrome of blood stasis with qi stagnation.Conclusion:Economic difficulty,history or expectation of surgery or interventional therapy,higher degree of Wagner may be the risk factors of depression in the patients with DF,the prevalence of depression may increase in syndrome of dampness-heat or blood stasis with qi stagnation.We should pay more attention to the mental status of patients with DF while giving vascular recanalization.
