摘 要:目的探讨慢性阻塞性肺疾病(COPD)患者合并贫血、低蛋白血症患病率及与年龄、性别的相关性。方法采用回顾性、便利抽样研究方法,选择2013年1月至2017年10月在我院诊治的COPD患者76例作为观察组,选择同期在我院体检中心的健康体检者76例作为对照组,记录两组入选者的性别、年龄等临床资料,调查贫血、低蛋白血症患病率并进行相关性分析。结果观察组的贫血与低蛋白血症为51.3%和40.8%,对照组为5.3%和3.9%,观察组显著高于对照组(P<0.05)。观察组中轻度贫血30例,中度贫血6例,重度贫血3例;蛋白质轻度缺乏25例,蛋白质中度缺乏3例,蛋白质重度缺乏3例。男性患者的贫血患病率显著高于女性(P<0.05),低蛋白血症在不同性别患者患病率对比中差异无统计学意义(P>0.05);不同年龄段患者的贫血与低蛋白血症患病率对比都有显著差异(P<0.05)。logistic回归分析显示性别、年龄也是导致COPD患者贫血的危险因素,年龄为导致低蛋白血症的危险因素(P<0.05)。结论COPD患者合并贫血、低蛋白血症患病率比较高,性别、年龄为导致贫血的危险因素,年龄为导致低蛋白血症的危险因素,应积极进行预防性管理。Objective To investigate the correlation between the incidences of anemia and hypoproteinemia and age and sex in the patients with chronic obstructive pulmonary disease (COPD). Methods With a convenient sampling method, 76 COPD patients who were treated in our hospital from January 2013 to October 2017 were selected as the observation group and the other 76 healthy persons who received the physical examination in our hospital at the corresponding time period were selected as the control group. The clinical data, such as the gender and age, and the incidences of anemia and hypoproteinemia were recorded for the two groups, and correlation analysis was conducted. Results The incidences of anemia and hypoproteinemia were respectively 51.3% and 40.8% in the observation group, and respectively 5.3% and 3.9% in the control group, which were significantly higher in the observation group than the control group (P<0.05). In the observation group, 30 patients suffered from mild anemia, 6 from moderate anemia, 3 from severe anemia, 25 from mild protein deficiency, 3 from moderate protein deficiency, and 3 from severe protein deficiency. The incidence of anemia in the male patients was significantly higher than that of the female ones (P<0.05). There was no significant difference in the incidences of hypoproteinemia between different sexes (P>0.05). The incidences of anemia and hypoproteinemia in the different age groups were significantly different (P<0.05). Logistic regression analysis showed that sex and age were risk factors for anemia in the COPD patients, and age was the risk factor for hypoproteinemia (P<0.05). Conclusion The incidences of anemia and hypoproteinemia in the COPD patients are relatively high. And sex and age are risk factors for anemia and age is a risk factor for hypoproteinemia. Therefore, preventive management should be carried out actively.
