摘  要:目的探讨单纯支气哮喘和支气管哮喘合并支气管扩张症患者呼出气NO(FeNO)水平的差异对比。方法选择2017年1月~2018年4月收治的单纯支气哮喘患者50例为对象,作为A组,另选取同时间段收治的支气管哮喘合并支气管扩张症患者50例为对象,作为B组;比较分析两组FeNO水平差异、第1秒用力呼气容积(FEV1)水平差异、最大呼气峰流速(PEF)水平差异、外周血清白介素-4(IL-4)水平差异、γ干扰素(IFN-γ)水平差异。结果单纯支气哮喘患者的FeNO水平[(26.45±10.30)ppb]明显低于支气管哮喘合并支气管扩张症患者,第1秒用力呼气容积水平[(2.86±0.31)L]明显高于支气管哮喘合并支气管扩张症患者,最大呼气峰流速水平[(3.95±0.25)L/min]明显高于支气管哮喘合并支气管扩张症患者,IL-4水平[(367.55±45.20)ng/L]明显高于支气管哮喘合并支气管扩张症患者,IFN-γ水平[(153.75±26.70)ng/L]明显低于支气管哮喘合并支气管扩张症患者,P <0.05。ROC曲线AUC=0.808,95%可信区间=0.840~0.968,截断点临界值=36.60ppb,敏感度为80.60%,特异度为72.10%。结论与单纯支气哮喘患者FeNO水平比较,支气管哮喘合并支气管扩张症患者FeNO水平更高,有重要临床应用价值。Objective To explore the differences in exhaled nitric oxide(FeNO) level between patients with simple bronchial asthma and bronchial asthma complicated with bronchiectasis. Methods 50 patients with simple bronchial asthma who were admitted and treated from January 2017 to April 2018 were selected as research objects and set as group A. 50 patients with bronchial asthma complicated with bronchiectasis who were admitted and treated in the same period were selected as research objects and set as group B. The differences in exhaled nitric oxide(FeNO)level, forced expiratory volume in the first second(FEV1), maximum peak expiratory flow rate(PEF), peripheral serum interleukin-4(IL-4) level and interferon-gamma(IFN-γ) were compared and analyzed between two groups. Results The exhaled nitric oxide level of patients with simple bronchial asthma was [(26.45±10.30) ppb], significantly lower than that in patients with bronchial asthma complicated with bronchiectasis. The forced expiratory volume in the first second of patients with simple bronchial asthma was [(2.86±0.31)L], significantly higher than that in patients with bronchial asthma complicated with bronchiectasis. The maximum peak expiratory flow rate of patients with simple bronchial asthma was [(3.95±0.25)L/min], significantly higher than that in patients with bronchial asthma complicated with bronchiectasis. IL-4 level of patients with simple bronchial asthma was [(367.55 ±45.20) ng/L],significantly higher than that in patients with bronchial asthma complicated with bronchiectasis. IFN-γ of patients with simple bronchial asthma was [(153.75±26.70) ng/L], significantly lower than that in patients with bronchial asthma complicated with bronchiectasis, P < 0.05.
