两点超声定位法在幼儿行颈内静脉穿刺置管术中的临床应用

摘　　要：目的比较两点超声定位法在幼儿行颈内静脉穿刺置管术中的临床效果。方法选取2017年10月~2019年1月于我院行择期手术且需颈内静脉穿刺的患儿80例,随机分为实验组和对照组,各40例。实验组行两点超声定位,对照组行体表标志,比较两组一次穿刺成功率,总成功率,误穿动脉,血肿、气胸等并发症发生率。结果实验组一次穿刺成功率及总成功率(87.50%vs100.00%)均高于对照组(62.50%vs82.50%),差异有统计学意义(P<0.05);实验组并发症发生率为0,低于对照组的12.50%,差异有统计学意义(P<0.05)。结论超声两点定位法可有效提高幼儿行颈内静脉穿刺置管成功率,降低误穿动脉或局部血肿的发生,具有较高的临床应用价值。Objective To compare the clinical effects of two-point ultrasound localization in the treatment of jugular vein catheterization in children. Methods 80 children with elective internal jugular vein puncture in our hospital from October 2017 to January 2019 were randomly divided into experimental group and control group, 40 cases each. The experimental group underwent two-point ultrasound localization, and the control group performed the body surface markers, comparing the success rate of one puncture, total success rate, arterial incision, hematoma, pneumothorax and other complications. Results The success rate and total success rate (87.50% vs 100.00%) of the experimental group were higher than those of the control group (62.50% vs 82.50%), the difference was statistically significant (P<0.05). The complication rate of the experimental group was 0 , which was lower than 12.50% of the control group,the difference was statistically significant (P<0.05). Conclusion Ultrasound two-point positioning method can effectively improve the success rate of jugular vein catheterization in children, reduce the occurrence of miscarriage of arteries or local hematoma, and has high clinical application value.
