祛瘀通痹方外洗联合双氯芬酸钠缓释胶囊治疗膝骨关节炎脾肾两虚、湿注骨节证临床观察

摘　　要：目的观察祛瘀通痹方外洗联合双氯芬酸钠缓释胶囊治疗膝骨关节炎(KOA)脾肾两虚、湿注骨节证的临床疗效。方法将83例KOA脾肾两虚、湿注骨节证患者按随机数字表法分为2组。对照组41例予双氯芬酸钠缓释胶囊治疗;治疗组42例在对照组基础上联合中药祛瘀通痹方外洗治疗。2组均治疗4周后统计疗效,比较2组治疗前后膝关节功能跛行、支撑、膝软、肿胀、爬楼梯及下蹲评分变化情况,比较2组治疗前后膝关节疼痛视觉模拟评分(VAS)变化情况。结果治疗组控显率66.67%,总有效率95.24%,对照组控显率39.02%,总有效率90.24%,2组总有效率比较差异无统计学意义(P>0.05),但治疗组控显率明显高于对照组(P<0.05)。2组治疗后膝关节功能跛行、支撑、膝软、肿胀、爬楼梯及下蹲评分与本组治疗前比较均明显升高(P<0.05),且治疗组治疗后膝软评分明显高于对照组(P<0.05)。2组治疗后膝关节疼痛VAS评分与本组治疗前比较均明显降低(P<0.05),且治疗组明显低于对照组(P<0.05)。结论祛瘀通痹方外洗联合双氯芬酸钠缓释胶囊治疗KOA脾肾两虚、湿注骨节证临床疗效确切,可明显改善患者临床症状,促进膝关节功能恢复,缓解疼痛,提高疗效。Objective To observe the clinical effects of Quyu Tongbi prescription external washing combined with diclofenac sodium sustained-release capsule in the treatment of knee osteoarthritis(KOA) with spleen and kidney deficiency and wet injection of bone joint syndrome. Methods 83 KOA patients with spleen and kidney deficiency and wet injection of bone joint syndrome were divided into two groups according to the random number table method. 41 cases in the control group were treated by diclofenac sodium sustained-release capsules;42 cases in the treatment group were treated by Quyu Tongbi prescription external washing on the basis of the control group. The curative effects of the two groups were evaluated after the treatment for 4 weeks. The changes of knee limp, support, knee weakness, swelling, stair climbing, squatting scores and the visual analogue score(VAS) of knee pain before and after treatment were compared between the two groups. Results The control rate of the treatment group was 66.67%, the total effective rate was 95.24%, the control rate of the control group was 39.02%, and the total effective rate was 90.24%. There was no significant difference in the total effective rate between the two groups(P>0.05), but the control rate of treatment group was significantly higher than that of control group(P<0.05). After treatment, the scores of knee limp, support, knee weakness, swelling, stair climbing and squatting in two groups were significantly higher than those before treatment(P<0.05), and the scores of knee softness in the treatment group were significantly higher than those in the control group(P<0.05). The VAS score of knee pain after treatment in two groups was significantly lower than that before treatment(P<0.05), and the treatment group was significantly lower than that in the control group(P<0.05). Conclusion Quyu Tongbi prescription external washing combined with diclofenac sodium sustained-release capsule in the treatment of KOA with with spleen and kidney deficiency and wet injection of bone joint
