针药联用治疗阻塞性睡眠呼吸暂停综合征合并原发性高血压临床疗效观察

摘　　要：目的观察针药联用治疗阻塞性睡眠呼吸暂停综合征(OSAHS)合并原发性高血压的临床疗效。方法将82例OSAHS合并原发性高血压患者随机分为2组。对照组41例予常规西医治疗;治疗组41例在对照组治疗基础上予针刺联合中药化痰通气汤治疗。2组均10d为1个疗程,连续治疗2个疗程。比较2组治疗前后夜间睡眠呼吸监测、24h动态血压监测指标及Epworth嗜睡量表(ESS)评分。结果治疗组总有效率95.12%,对照组总有效率80.49%,治疗组疗效优于对照组(P<0.05)。治疗后2组呼吸暂停低通气指数(AHI)及ESS评分均降低(P<0.05),夜间最低血氧饱和度(LSaO2)均升高(P<0.05);且治疗组AHI及ESS评分均低于对照组(P<0.05),LSaO2高于对照组(P<0.05)。治疗后2组24h、日间、夜间平均收缩压、平均舒张压均降低(P<0.05),且治疗组均低于对照组(P<0.05)。结论针药联用治疗OSAHS合并原发性高血压能有效改善患者睡眠质量,降低血压。Objective To observe the clinical efficacy of acupuncture combined with medicine in the treatment of obstructive sleep apnea syndrome(OSAHS) with essential hypertension. Methods 82 patients with OSAHS complicated with essential hypertension were randomly divided into two groups. 41 cases in control group were treated by routine western medicine. 41 cases in treatment group were treated by acupuncture combined with Huatan Tongqi decoction on the basis of the control group. The two groups were treated for 10 days as a course of treatment, with two consecutive courses of treatment. The night sleep breathing monitoring, 24-hour ambulatory blood pressure monitoring and Epworth sleepiness scale(ESS) scores were compared between the two groups before and after treatment. Results The total effective rate was 95.12% in treatment group and 80.49% in control group. The clinical curative effect of the treatment group was better than that of the control group(P<0.05). After treatment, the apnea hypopnea index(AHI) and ESS scores in both groups were decreased(P<0.05), the lowest oxygen saturation at night(LSaO2) were increased(P<0.05), and the AHI and ESS scores in the treatment group were lower than those in the control group(P<0.05), and LSaO2 was higher than that in the control group(P<0.05). After treatment, the mean systolic blood pressure and mean diastolic blood pressure were decreased in the two groups at 24 h, daytime, nighttime(P<0.05), and the treatment group was lower than the control group(P<0.05). Conclusion Acupuncture combined with medicine can effectively improve sleep quality and reduce blood pressure in patients with OSAHS complicated with essential hypertension.
