阴式子宫切除术结合骶棘韧带固定术治疗阴道顶端脱垂的价值

摘　　要：目的 探讨阴式子宫切除术结合骶棘韧带固定术治疗阴道顶端脱垂的价值和意义。方法 选择2017年7月至2018年7月在该院治疗的阴道顶端脱垂80例患者为研究对象,将其随机分为对照组和观察组,每组40例。对照组采用阴式子宫切除术治疗,观察组采用阴式子宫切除术结合骶棘韧带固定术治疗。比较两组患者的治疗有效率、生活质量及性功能情况。结果 观察组和对照组患者治疗的总有效率分别为95.00%(38/40)和70.00%(28/40),两组比较,差异有统计学意义(χ^ 2=8.95, P <0.05)。治疗后,观察组盆底功能障碍性疾病症状问卷表(PFDI-20)评分低于对照组,盆底器官脱垂/尿失禁对性功能影响问卷简表12(PISQ-12)评分高于对照组,差异有统计学意义( P <0.05)。结论 阴道顶端脱垂的患者采用阴式子宫切除术结合骶棘韧带固定术方案治疗,不仅能够增加治疗的总有效率,同时可改善患者生活质量,效果显著,值得推广。Objective To explore the value and significance of vaginal hysterectomy combined with sacrospinous ligament fixation in the treatment of vaginal apex prolapse. Methods A total of 80 cases of vaginal apical prolapse patients treated in our hospital from July 2017 to July 2018 were selected.By random number table method,patients were divided into two groups (40 cases in control group and 40 cases in observation group).The patients who underwent vaginal hysterectomy alone were listed as control group.Patients who received vaginal hysterectomy combined with sacrospinous ligament fixation were included in the observation group.The effects of the curative effect,quality of life and sexual function were compared. Results In this study,the total effective rates of the observation group and the control group were 95.00%(38/40) and 70.00 %(28/40) respectively.The difference of total effective rates of the observation group and the control group was significantly significant (χ 2=8.95, P <0.05).The score of symptoms questionnaire of pelvic floor dysfunction diseases (FDI-20) of the observation group was significantly lower than that of the control group ( P <0.05). The PISQ-12 scores of prolapse/urinary incontinence questionnaire on sexual function of the observation group were significantly higher than that of control group ( P <0.05). Conclusion Vaginal hysterectomy combined with sacrospinous ligament fixation can not only increase the total effective rate of treatment,but also improve the quality of life of patients,the effect is remarkable,which is worthy of promotion.
