摘　　要：目的分析系统性红斑狼疮(SLE)患者外周全血中T淋巴细胞(Th)22细胞的含量,其关键转录因子芳香烃受体(AhR)的表达量及其外周血清中白细胞介素(IL)-22的水平,探讨Th22细胞在SLE的发生、发展中的作用机制。方法研究对象为SLE患者46例,健康对照组36例,采用流式细胞术(FCM)检测外周血中Th22细胞的比值,RT-PCR技术检测外周血中AhRmRNA的相对表达量,酶联免疫吸附试验(ELISA)法检测外周血清中IL-22水平。结果活动期SLE患者组和非活动期SLE患者组Th22细胞比值、AhRmRNA及IL-22水平均显著低于健康对照组,差异有统计学意义(P<0.05),活动期SLE患者组Th22细胞的比值显著低于非活动期SLE患者组,差异有统计学意义(P<0.05),但两组间AhRmRNA、IL-22的表达水平差异无统计学意义(P>0.05),SLE患者Th22细胞比例与AhRmRNA及IL-22的水平均呈正相关,Th22细胞比例与系统性红斑狼疮疾病活动指数(SLEDAI)呈负相关,但IL-22水平和AhRmRNA水平均与SLEDAI无相关性。结论SLE患者Th22细胞表达降低,提示改变或促进Th22细胞分化的上游或下游通路可能减轻SLE的免疫炎性反应,Th22水平可以作为SLE疗效的辅助标志物。Objective To analyze the level of T-cell 22 (Th22) cells and its key transcription factors arylhydrocarbon receptors(AhR) in peripheral blood of systemic lupus erythematosus (SLE) patients,the level of interleukin (IL)-22 in serum,and to investigate the mechanism of Th22 cells in the process of SLE. Methods The subjects were 46 SLE patients and 36 healthy controls.The proportion of Th22 cells was detected by flow cytometry(FCM),the expression of the AhR mRNA from peripheral blood was detected by RT-PCR,and the level of IL-22 in serum was detected by enzyme-linked immunosorbent assay (ELISA). Results Th22 cell ratio,AhR mRNA and IL-22 levels in active SLE patients and inactive SLE patients were significantly lower than those in healthy control group,and the differences were statistically significant (P<0.05).Th22 cell ratio in active SLE patients was significantly lower than that in inactive SLE patients,and the differences were statistically significant (P<0.05),while there was no statistically significantly difference on the levels of AhR mRNA and IL-22 between active SLE group and inactive SLE group (P >0.05).Th22 cell ratio in SLE group was positively associated with AhR mRNA and IL-22,and Th22 cells in SLE group was negatively associated with systemic lupus erythematosus disease activity index (SLEDAI),but the level of IL-22 and AhR mRNA had no correlation with SLEDAI. Conclusion Th22 cells are lowly expressed in patients with SLE,which indicates that changing or promoting the upstream or downstream of Th22 cells differentiation would be likely to reduce the immunoinflammatory response,and the level of Th22 might be the auxiliary index in the treatment of SLE.
