清洁手术围手术期抗菌药物预防应用规范化管理效果评价

摘　　要：目的:通过调查某三级甲等医院在实施规范化管理前、后清洁手术围手术期预防应用抗菌药物的情况,评价抗菌药物合理使用规范化管理的效果,为制定有针对性的抗菌药物规范化管理措施提供依据。方法:选取2016年1~3月清洁手术出院病例为干预前组,选取2016年10~12月清洁手术出院病例为干预后组,对规范化管理前、后围手术期抗菌药物预防应用的合理性变化进行对照研究。结果:经过规范化管理后,全院清洁手术抗菌药物使用合理率由24.27%提高至76.90%,清洁手术预防应用抗菌药物适应证选择、首剂用药时间、品种选择和用药疗程的合理率均显著提高,差异均具有统计学意义(P <0.01)。结论:实施以行政干预、专项督查、药师参与、宣传教育等相结合的规范化管理措施对提高清洁手术围手术期抗菌药物应用的合理化水平效果显著。Objective: To offer a basis for the formulation of specific antimicrobial management measures by the investigation of the rational preventive use of antimicrobial agents during the perioperative period of clean operation before and after the implementation of standardized management in a 3 A hospital. Methods: Selected the eligible cases totally of clean incision operations which were discharged into pre-intervention group(from January 2016 to March 2016) and post-intervention group(from October 2016 to December 2016), and comparing the rational changes of preventive use antimicrobials during the perioperative period before and after standardized management between the pre-intervention group with the post-intervention group. Results: The rational use of antimicrobial agents in clean operation increased from 24.27%to 76.90% after implementing the standardized management. And the rational rate of indication choice, the first dose time,the species selection and the course of medication also were significantly improved and the differences were statistically significant(P < 0.01). Conclusion: The implementation of standardized management measures which was combined with administrative intervention, special inspection, pharmacists’ participation, publicity and education significantly improved the level of rational use of antibacterials during the perioperative period of clean surgery.
