锁定钢板与带锁髓内钉用于胫腓骨不稳定性骨折中对感染率、愈合时间等的影响

摘　　要：目的:研究胫腓骨不稳定性骨折患者临床接受锁定钢板、带锁髓内钉治疗的不同效果。方法:选择87例于2017年4月-2018年4月入住我院的胫腓骨不稳定性骨折,患者根据手术方式不同分为观察组、对照组,观察组有44例接受带锁髓内钉手术治疗,对照组有43例接受锁定钢板治疗,比较2组手术效果。结果:观察组手术时间、术后患肢消肿时间、术中出血量、骨折愈合时间均与对照组差异明显(P<0.05);观察组术后1个月膝踝功能评分高于对照组(P<0.05);观察组术后并发症发生率为15.91%,对照组术后并发症发生率20.93%(P>0.05)。结论:锁定钢板与带锁髓内钉治疗胫腓骨不稳定性骨折均有良好效果,但带锁髓内钉能够保证骨折更高质量恢复,提升患者关节功能,更值得在临床推广。Objective:To study the different effects of locking plate and interlocking intramedullary nail in the treatment of unstable tibiofibular fracture.Methods:87 cases of unstable fracture of tibia and fibula admitted to our hospital from April 2017 to April 2018 were selected and divided into observation group and control group according to different surgical methods.In the observation group,44 cases were treated with interlocking intramedullary nail,while 43 cases in the control group were treated with locking plate.Results:There were significant differences between the observation group and the control group in operation time,limb detumescence time,intraoperative bleeding volume and fracture healing time(P<0.05).The knee and ankle function score of observation group was higher than that of control group one month after operation(P<0.05).The incidence of postoperative complications was 15.91%in the observation group and 20.93%in the control group(P>0.05).Conclusion:Locking plate and interlocking intramedullary nail had good effects in the treatment of unstable tibia and fibula fractures,but interlocking intramedullary nail could ensure a higher quality of fracture recovery and improve the joint function of patients,which was more worthy of clinical promotion.
