闭合复位与切开复位克氏针内固定术治疗儿童肱骨髁上骨折效果比较

摘　　要：目的:对比闭合复位与切开复位克氏针内固定术治疗儿童肱骨髁上骨折的效果.方法:选取我院2016年1月-2018年1月收治的儿童肱骨髁上骨折患者52例为研究对象,根据患者所用手术方案划分为切开复位组与闭合复位组,切开复位组实施切开复位克氏针内固定术,闭合复位组实施闭合复位克氏针内固定术治疗,对比2组患者治疗后临床指标情况及治疗效果.结果:切开复位组与闭合复位组患者手术时间、术中出血量等手术指标相比,差异有统计学意义(P<0.05);闭合复位组患者治疗优良率92.31%,略高于切开复位组患者88.46%,但无统计学意义(P>0.05);闭合复位组患者并发症发生率7.69%,与切开复位组23.08%相比,差异存在统计学意义(P<0.05).结论:闭合复位克氏针内固定术治疗儿童肱骨髁上骨折出血量少、术后恢复快且并发症发生率低,有一定推广应用价值.Objective: To compare the effect of closed reduction and open reduction with Kirschner wire internal fixation in the treatment of supracondylar fracture of humerus in children. Methods: Fifty - two children with supracondylar fracture of humerus admitted to our hospital from January 2016 to January 2018 were selected as the research object. According to the surgical plan of the patients, they were divided into open reduction group and closed reduction group, the open reduction group was treated with open reduction Kirschner wire internal fixation, and the closed reduction group was treated with closed reduction Kirschner wire internal fixation. The clinical indexes and therapeutic effects of the two groups were compared. Results: There was a significant difference between the open reduction group and the closed reduction group in operation time, intraoperative blood loss and other operation indexes ( P < 0. 05 ). The excellent and good rate of treatment in closed reduction group was 92. 31 % slightly higher than 88. 46 % in open reduction group, but there was no statistical significance ( P > 0. 05 ). The incidence of complications in the closed reduction group was 7. 69 % compared with 23.08 % in the open reduction group ( P <0.05 ). Conclusion: Closed reduction and Kirschner wire internal fixation for the treatment of supracondylar fracture of humerus in children has less bleeding, quick recovery and low incidence of complications, which has certain popularization and application value.
