摘　　要：目的探讨维持性血液透析(MHD)患者握力与营养指标的相关性。方法采用横断面调查研究,收集2018年3—9月在河池市人民医院透析中心规律血液透析的124例患者的临床资料及相关生化指标,同时测量其握力、身高、体重。采用Spearman相关性分析及多元线性回归分析MHD患者握力与营养指标的相关性。结果124例MHD患者的平均握力(22.95±7.81)kg,平均相对握力(1.029±0.135)kg,其中男80例(64.5%),女44例(35.5%)。性别、身高、体重、血红蛋白(Hb)、血清白蛋白(Alb)、血肌酐(Scr)与握力呈正相关(P<0.01),年龄、透析年龄、尿素清除指数(Kt/V)、空腹血糖(FPG)与握力呈负相关(P<0.01);多元线性回归分析显示,调整体重指数后的握力即相对握力的独立影响因素有:性别、年龄、Hb、Alb、Scr、FPG,其中性别与相对握力的回归系数最高(β=0.074),Scr和FPG仅与男性患者相对握力有关。结论MHD患者握力与营养指标密切相关,是潜在的营养状态标记物,值得在临床实践和研究中进一步推广应用。Objective To investigate the correlation between handgrip strength and nutritional indicators in main- tenance hemodialysis ( MHD) patients.Methods A cross - sectional survey was conducted to collect the clinical data and related biochemical parameters of 124 patients receiving regular MHD in the dialysis center of The People's Hospital of Hechi from March to September 2018.Their handgrip strength,height,and weight were also measured.Spearman correla- tion analysis and multiple linear regression were used to analyze the correlation between handgrip strength and nutritional indicators in MHD patients.Results The average handgrip strength of 124 patients was ( 22. 95 ± 7. 81) kg,relative handgrip strength was ( 1. 029 ± 0. 135) kg.There were 80 males ( 64. 5%),and 44 females ( 35. 5%).Height,weight, hemoglobin,serum albumin,serum creatinine were significantly positively correlated with handgrip strength ( P < 0. 001);while age,duration of dialysis,urea clearance index,and fasting blood glucose were significantly negatively correlated with handgrip strength ( P < 0. 001).Multiple linear regression analysis showed that the independent influencing factors of handgrip strength after adjusting body mass index ( BMI) were gender,age,hemoglobin,serum albumin,serum creati- nine,and fasting blood glucose;and the regression coefficients between sex and relative handgrip strength was the highest (β= 0. 074),while serum creatinine and fasting blood glucose were only associated with the relative handgrip strength in male patients.Conclusion Handgrip strength of MHD patients is closely correlated to nutritional indicators and is a reli- able nutritional marker.
