摘　　要：目的 探讨维持性血液透析(maintenance hemodialysis,MHD)患者治疗不依从性的原因。方法 选取2018年9月至2019年1月在本市某三级甲等医院血液净化中心进行MHD治疗且医护人员评估治疗过程存在不依从现象的患者。采用质性研究中的现象学方法,以面对面的半结构访谈法,对15例MHD患者治疗不依从性的原因进行深入访谈。结果 MHD患者治疗不依从性的原因主要有3个:对相关知识了解不够、心存侥幸、顺其自然。结论 良好的维持性血液透析治疗方案固然重要,但若不能很好依从也会失败,医护人员应重视其治疗不依从性的原因,提供多层次、延续性、个体化、全方位的健康教育和指导,构建社会、家庭、同伴共同支持网,加强心理辅导。Objective To investigate the causes of non-compliance in patients undertakine maintenance hemodialysis(MHD).Methods From September 2018 to January 2019,MHD treatment was carried out in the blood purification center of a tertiary first-class hospital in our city and medical staff were evaluated.After a long in-depth communication with 15 non-compliance MHD patients,We have done the research by adopting the phenomenological qualitative method to find out the causes of non-compliance.Results There are three main reasons for the non-compliance in MHD patients,including insufficient knowledge of relevant maintenance haemodialysis,fluke mind,and with no care of future consequences.Conclusion A good maintenance hemodialysis treatment program is very important,but success will be unachievable if not conducted properly.So,medical staff should pay more attention to find out the reasons for non-compliance treatment,and provide a multilayered,continuous,individualized,and comprehensive health plan.Education and guidance is essential to build a social,familial and partnering support network and to strengthen psychological counseling.
