摘　　要：目的观察梅花针叩刺联合易化技术对卒中痉挛性偏瘫平衡功能的影响。方法选取2014年3月—2018年6月上海中医药大学附属第七人民医院神经康复科收治的90例老年脑卒中痉挛性偏瘫患者,随机分成Bobath康复组,梅花针叩刺康复组和梅花针叩刺联合Bobath康复组,每组30例。3组均采用常规内科治疗,Bobath康复组采用Bobath技术治疗,梅花针叩刺康复组采取基于Rood理论的梅花针叩刺治疗,梅花针叩刺联合Bobath康复组予以梅花针叩刺联合Bobath技术治疗。评估并比较3组在治疗前与治疗后2周、1个月、3个月、6个月的下肢MAS评分、Berg评分和BI评分。结果治疗1个月后梅花针叩刺联合Bobath康复组下肢MAS评分、Berg评分与Bobath康复组或梅花针叩刺康复组相比明显改善,差异具有统计学意义(P<0.05)。治疗3个月后梅花针叩刺联合Bobath康复组BI评分与Bobath康复组或梅花针叩刺康复组相比显著改善,差异具有统计学意义(P<0.05)。结论梅花针叩刺联合易化技术可有效改善老年脑卒中后痉挛期偏瘫患者下肢痉挛状态和平衡功能,提高患者的生活质量。Objective To observe the effect of plum blossom needle tapping plus technique on the balance function of the elderly with spastic hemiplegia.Methods 90 elderly with spastic hemiplegia after stroke hospitalized during the period from Mar.,2014 to June,2018 were randomly divided into 3 groups: Bobath technology group,plum blossom needle tapping group and plum blossom needle tapping plus Bobath technology group,30 in each,the 3 different treatment were applied to the cases in the 3 groups respectively;the Modified Ashworh Scale (MAS) for lower limbs,Berg scale and BI scale were applied to the cases at the time points of before treatment,2 weeks after treatment,1 month after treatment,3 months after treatment and 6 months after treatment and the scores were compared.Results The scores of MAS,Berg 1 months after treatment and the scores of BI 3 months after treatment in in plum blossom needle tapping plus Bobath technology group were obviously improved,and the differences from Bobath technology group and plum blossom needle tapping group were of statistical significance (P<0.05).Conclusions Plumblossomneedle tapping plus facilitation technique can effectively improve lower limb spasm and balance function of the elderly with spastic hemiplegia after stroke and promote the quality of life.
