老年人股骨转子间骨折PFNA和DHS内固定术疗效的临床研究

摘　　要：目的探究老年人股骨转子间骨折PFNA和DHS内固定术的临床疗效。方法将2017年5月-2018年8月期间在该院接受诊治的110例老年股骨转子间骨折患者作为研究对象,并将其分为两组,PFNA组患者采用PFNA内固定术进行治疗,DHS组患者采用DHS内固定术进行治疗,治疗结束后,对两组患者的各项手术指标及术后恢复情况进行调查。结果 PFNA组患者各项手术指标均显著优于DHS组患者,数据经检验差异有统计学意义(t=9.32、13.29、22.95、17.83、8.25,P<0.05)。PFNA组中恢复效果为优、良、中、差的患者分别为20例、23例、10例、2例,优良率为78.18%;DHS组中恢复效果为优、良、中、差的患者分别为17例、22例、11例、5例,优良率为70.91%,数据经检验差异有统计学意义(χ^2=7.92、7.45、9.13、6.95、10.33,P<0.05)。讨论与DHS内固定术相比,PFNA内固定术治疗老年股骨转子间骨折,对患者造成的伤害相对较小,后期恢复效果更为显著,值得推广。Objective To investigate the clinical efficacy of PFNA and DHS internal fixation for the treatment of intertrochanteric fractures in the elderly. Methods A total of 110 elderly patients with intertrochanteric fractures who were treated in our hospital from May 2017 to August 2018 were enrolled in the study. They were divided into two groups. Patients in the PFNA group were treated with PFNA internal fixation. Patients in the DHS group were treated with DHS internal fixation. After the treatment, the surgical indexes and postoperative recovery of the two groups were investigated. Results All the surgical indexes of the PFNA group were significantly better than those of the DHS group.The data were statistically significant(t=9.32, 13.29, 22.95, 17.83, 8.25, P<0.05). In the PFNA group, the patients with excellent, good, moderate, and poor recovery were 20 cases, 23 cases, 10 cases, and 2 cases, respectively, and the rate of excellent and good was 78.18%. In the DHS group, the recovery was excellent, good, moderate, and 17 cases 22 cases, 11 cases, and 5 cases, the excellent and good rate was 70.91%, and the data were statistically significant(χ^2=7.92,7.45, 9.13, 6.95, 10.33, P <0.05). Conclusion Compared with DHS internal fixation, PFNA internal fixation for the treatment of intertrochanteric fractures in the elderly, caused relatively small damage to the patient, and the recovery effect is more significant in the later stage. It is worthy of promotion.
