摘  要:目的探讨瘦素水平对预测妊娠期糖尿病(GDM)的临床价值。方法选取石家庄市第四医院2015年12月-2016年12月收治的GDM患者60例为GDM组,以同期健康妊娠者60例为对照组。比较两组患者血清瘦素、炎症因子和氧化还原酶的水平,并进行瘦素水平对GDM影响的回归分析。结果与对照组相比,GDM组血清瘦素、TNF-α和IL-6水平明显升高(P<0. 01),丙二醛(MDA)水平明显升高,超氧化物歧化酶(SOD)明显降低(P<0. 01);相关性分析显示,GDM发生的独立危险因素为瘦素水平升高及SOD降低,且瘦素水平与BMI呈正相关。结论 GDM患者存在明显的瘦素水平变化和炎症反应,这可能与患者体内氧化应激反应有关,瘦素水平对预测GDM有重要价值。Objective Investigate the clinical value of leptin level in predicting gestational diabetes mellitus ( GDM ). Methods 60 GDM patients admitted to the fourth hospital of shijiazhuang from December 2015 to December 2016 were selected as the GDM group,and 60 healthy pregnant women in the same period were selected as the control group. The levels of serum leptin,inflammatory factors and oxidation- reduction enzyme in the two groups were compared,and the effects of leptin levels on GDM were analyzed by regres- sion. Results Compared with the control group,serum leptin,TNF-αand IL-6 levels were significantly increased in the GDM group ( P< 0. 01). Malondialdehyde ( MDA) level was significantly increased,but superoxide dismutase ( SOD) level was significantly decreased ( P< 0. 01). Correlation analysis showed that the increased leptin level and decreased SOD level were the independent risk factor of GDM,and leptin level was positively correlated with body mass index ( BMI). Conclusion there were significant changes in leptin levels and inflam- matory factors in patients with GDM,which may be related to oxidative stress response in patients. Leptin level was of great value in predic- ting GDM.
