摘 要:目的观察韩氏内异止痛汤对EMs大鼠异位内膜组织病理学改变的影响,探索雌二醇(E2)、雌激素受体(ER)、芳香化酶P450,阐明韩氏内异止痛汤抑制异位子宫内膜增生的机制。方法采用自体移植法建立EMs大鼠模型,选取57只SD大鼠进行自体移植手术,并于手术后4周第2次剖腹,观察异位内膜病灶形态变化,将造模成功的40只EMs大鼠随机分为内异止痛汤高剂量组、内异止痛汤低剂量组、达那唑阳性药物组和模型对照组,每组10只,另设假手术组8只。各组大鼠分别给予对应的药物及生理盐水灌胃,治疗4周后处死并取其异位内膜组织及血清。应用酶联免疫法(ELISA)检测各组大鼠给药后血清中E2以及异位内膜组织匀浆液中ER和芳香化酶P450的变化。结果对各组大鼠治疗4周后剖腹观察发现,各治疗组子宫内膜异位病灶的体积比治疗前明显缩小,模型组治疗前后子宫内膜异位病灶的体积无明显变化。酶联免疫法结果显示,各药物治疗组大鼠经治疗后,与模型组比较,E2、ER和芳香化酶P450含量明显降低。与中药低剂量组相比,中药高剂量组对EMs模型大鼠血清E2及异位组织中ER和芳香化酶P450含量降低更为显著(P <0.05)。结论韩氏内异止痛汤可有效下调EMs模型大鼠血清雌激素E2和组织雌激素受体水平,从而使阻断了雌激素受体作为信号传导者与雌激素结合活化及其与雌激素效应元件(ERE)结合,使异位病灶细胞生长和增殖得到抑制。Objective To observe the effect of Hanshi Neiyi Zhitong Decoction on the expression of E2 and aromatase P450 in Endometriosis Rat model, in order to explore the mechanism of Hanshi Neiyi Zhitong Decoction inhibiting the proliferation of ectopic endometrium. Methods EMs rat model was established by autologous transplantation. 57 SD rats were selected for autologous transplantation. The morphological changes of ectopic endometrial lesions were observed after the second laparotomy 4 weeks after operation. The successful 40 EMs rats were randomly divided into high dose group, low dose group, danazole positive drug group and model control group, 10 in each group, and 8 in the sham operation group. The rats were given the corresponding drugs and saline irrigation, and after 4 weeks of treatment, the rats were sacrificed and their ectopic endometrium tissues and serum were taken. The changes of ER and aromatase P450 in serum E2 and endometrial tissue homogenase were detected by enzymelinked immunoassay(ELISA). Results The volume of endometriosis lesions in each treatment group was significantly smaller than that before treatment, while the volume of endometriosis lesions in model group had no significant change before and after treatment. After treatment, the contents of E2, ER and aromatase P450 in each drug treatment group were significantly lower than those in the model group. Compared with the low-dose group, the decrease in the content of ER and aromatase P450 in the serum E2 and heterotopic tissues of the EMs model was significantly lower in the high dose group(P < 0.05). Conclusion Han’s Neiyi Zhitong Decoction can effectively reduce the serum estrogen E2 and estrogen receptor level in the EMs model, thus blocking the activation of estrogen receptor as signal transducer and its binding with estrogen effect element(ERE), and inhibiting the growth and proliferation of ectopic lesions.
