三黄汤治疗痰湿热结型2型糖尿病43例

摘　　要：目的:观察三黄汤治疗痰湿热结型2型糖尿病的临床疗效。方法:将86例痰湿热结型2型糖尿病患者按照随机数字表法分为对照组和治疗组,每组各43例。对照组给予胰岛素联合二甲双胍治疗,治疗组在对照组的基础上加用三黄汤口服。比较两组患者血糖达标时间、胰岛素酶使用剂量及治疗前后临床症状积分、体质量指数(body mass index,BMI),检测两组患者治疗前后空腹血糖(Fasting blood glucose,FBG)、空腹胰岛素(fasting insulin,FINS)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)、胰岛素抵抗指数(HOMA-IR)、三酰甘油(three acyl glycerin,TG)、血清总胆固醇(totalcholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)和高密度脂蛋白胆固醇(high density lipoproteincholesterol,HDL-C)水平。结果:两组患者治疗后临床症状积分、FBG、FINS、HbA1c、HOMA-IR、TC、TG、LDL-C、BMI低于本组治疗前,且治疗组低于对照组,差异具有统计学意义(P<0.05)。两组患者血糖达标时间比较,差异无统计学意义(P>0.05)。治疗组每日胰岛素使用剂量低于对照组,差异具有统计学意义(P<0.05)。结论:三黄汤能显著降低痰湿热结型2型糖尿病患者的血糖、血脂水平,改善患者临床症状,减少胰岛素用量。Objective: To observe the clinical curative effect of Three Yellows Decoction on type II diabetes mellitus with phlegmdampness- heat accumulation syndrome. Methods: A total of 86 cases of type II diabetes mellitus with phlegm-dampness-heat accumulation syndrome were divided into the control group and the treatment group according to random number table method,with 43 cases in each group. The control group was treated with insulin combined with metformin,while the treatment group was treated with Three Yellows Decoction on the basis of the control group. The blood glucose time up to the standard,insulin dosage, clinical symptom score,body mass index ( BMI) before and after the treatment were compared between the two groups. The levels of fasting blood glucose ( FBG),fasting insulin ( FINS),glycosylated hemoglobin ( HbA1c) and insulin resistance index ( HOMA-IR),three acyl glycerin ( TG),total cholesterol ( TC),low density lipoprotein cholesterol,( LDL-C) high density lipoprotein cholesterol ( HDL-C) before and after the treatment were measured. Results: After the treatment,the scores of clinical symptoms,FBG,FINS,HbA1c,HOMA-IR,TC,TG,LDL-C and BMI of the two groups were lower than those before the treatment, and the treatment group was lower than the control group,all the differences were statistically significant ( P < 0. 05). However,there was no significant difference in the time of blood sugar reaching the standard between the two groups ( P > 0. 05). The daily dose of insulin in the treatment group was lower than that in the control group,and the difference was statistically significant ( P < 0. 05). Conclusion: Three Yellows Decoction has a remarkable efficacy on type II diabetes mellitus with phlegm-dampness-heat accumulation syndrome,can significantly reduce the patients' level of blood sugar and lipid improve the clinical symptoms and reduce the dosage of insulin.
