摘　　要：目的:评价植入性心脏起搏器治疗中西医结合临床路径实施效果。方法:采用文献调研、专家咨询、回顾性调研等方法制定具有中医特色的植入性心脏起搏器治疗中西医结合诊疗方案及临床路径,通过在我科行历史对照研究,完成入组病人的住院时间、围术期时间、治疗后住院总费用、药费、材料费、并发症及不良反应发生率、抗生素使用情况、中医证候疗效及满意度评定,经整理后建立数据库,用SPSS17.0软件包进行数据分析,以了解本路径的有效性、可行性,评价此路径在卫生经济学、疗效等方面的优势。结果:路径组的材料费、材料费占总费用比例、抗生素使用天数以及治疗后中医证候积分均低于常规组(P<0.05),而药费、日均药费、药占比均高于常规组高(P<0.05),住院时间、围术期时间、住院总费用、日均住院总费用与常规组比较,差异无统计学意义(P>0.05)。两组并发症及不良反应发生率、满意度两组比较,差异无统计学意义(P>0.05)。结论:通过临床路径的管理,降低住院费用,改善临床症状,提高满意度,减少抗生素使用,控制并发症及不良反应发生。Objective:To evaluate the effect of implanted pacemaker in treating clinical path of integrated traditional Chinese and western medicine.Methods:We adopted the method of literature investigation,expert consultation,and retrospective survey to develop diagnosis and treatment plan,and collected the data:hospitalization time,perioperative time,total hospitalization cost,drug cost,material cost,occurrence of adverse reactions,use of antibiotics,efficacy and satisfaction.Then we established a database,used SPSS17.0 software package for data analysis,to understand the effectiveness and feasibility of the path,evaluate the effect of this path in health economics,such as advantages,the formation of implantable cardiac pacemakers treatment combined with traditional Chinese and western medicine diagnosis and treatment plan.Results:The cost of material,the days of antibiotic use and TCM syndromes after treatment were lower than those in the conventional group(P<0.05).Hospitalization time,perioperative time,total cost of hospitalization,total daily hospitalization cost was equal to routine group(P>0.05).There was no significant difference in complication,incidence of adverse reactions and satisfaction between the two groups(P>0.05).Conclusion:Through the management of clinical pathways,we can reduce hospital costs,improve clinical symptoms,reduce the use of antibiotics,control complications and adverse reactions occur.
