针刀疗法联合整脊手法治疗颈源性头痛的疗效观察

摘　　要：目的:观察针刀疗法联合整脊手法治疗颈源性头痛的临床疗效。方法:将69例颈源性头痛患者随机分为3组,每组23例。治疗组患者予针刀联合整脊手法治疗,对照A组患者予针刀治疗,对照B组患者予整脊手法。结果:治疗后3组患者VAS评分、头痛频率、持续时间均较治疗前降低,差异均有统计学意义(P<0.05);治疗后3组比较,治疗组患者头痛VAS评分、频率、持续时间均低于对照A组、对照B组,差异均有统计学意义(P<0.05或P<0.01);治疗组总有效率为86.96%(20/23),对照A组总有效率为78.26%(18/23),对照B组总有效率为60.87%(14/23),3组比较,差异有统计学意义(P<0.05);完成治疗3个月后复发率治疗组为10.00%(2/20),对照A组为33.33%(6/18),对照B组为50.00%(7/14),3组比较,差异有统计学意义(P<0.05)。结论:针刀疗法联合整脊手法可提高颈源性头痛疗效,且复发率低于单一针刀疗法、整脊手法。Objective:To observe the clinical efficacy of acupotomy combined with chiropractic manipulation for cervicogenic headache.Methods:69 patients with cervicogenic headache were randomly divided into 3 groups,23 in each group.Patients in the treatment grou were given acupotomy and chiropractic manipulation.Patients in the control group A were given acupotomy,and patients in the control group B were given chiropractic manipulation.Results:After treatment,the VAS score,headache frequency and duration were significantly lower than those before treatment(P<0.05).After treatment,the VAS score,frequency and duration of headache in the treatment group were lower than those in the control group A and B(P<0.05 or P<0.01).The total effective rate of the treatment group was 86.96%(20/23),and the total effective rate of the control group A was 78.26%.(18/23),the total effective rate of control group B was 60.87%(14/23),the difference was statistically significant in the three groups(P<0.05).The recurrence rate was 10.00%in the treatment group after 3 months of treatment(2/20),control group A was 33.33%(6/18),and control group B was 50.00%(7/14),the difference between the three groups was statistically significant(P<0.05).Conclusion:Acupotomy combined with chiropractic manipulation can improve the curative effect of cervicogenic headache,and the recurrence rate was lower than that of single acupotomy and chiropractic manipulation.
