摘　　要：目的动静脉内瘘是维持性血液透析患者的首选血管通路。本研究回顾性分析湖南省娄底地区血液透析患者的临床资料,了解血管通路现状。方法回顾性分析湖南省娄底地区三家大型血液透析中心自2017年1月1日至2017年12月31日新导入及维持性透析患者的基本资料、血管通路选择和并发症。结果维持性血液透析患者486例,选择自体动静脉内瘘80.25%、带涤纶套带隧道导管18.93%。新导入患者86例,首次透析患者选择无隧道无涤纶套导管83.72%,AVF5.81%。AVF并发症发生率40.26%,其中血栓和血栓相关血管狭窄发生率24.36%。结论维持性血液透析患者绝大多数选择AVF,而新导入透析患者中选择无隧道无涤纶套导管占比大。Objective: Arteriovenous fistula is the preferred vascular access for maintenance hemodialysis patients. This study retrospectively analyzed the clinical data of hemodialysis patients in Loudi area of Hunan Province and understood the current status of vascular access. Methods: The basic data, vascular access options and complications of newly introduced and maintenance dialysis patients from January 1, 2017 to December 31, 2017 were retrospectively analyzed in three large hemodialysis centers in Loudi, Hunan Province. Results: Of the 486 patients with maintenance hemodialysis, 80.25% of autologous arteriovenous fistulas and 18.93% of tunnels with polyester sheaths were selected. 86 patients were newly introduced, and the first dialysis patients chose 83.72% without tunnel and no polyester sleeve, and AVF 5.81%. The incidence of AVF complications was 40.26%, and the incidence of thrombosis and thrombosis-related stenosis was 24.36%. Conclusion: The majority of patients with maintenance hemodialysis choose AVF, and the proportion of non-tunnel-free polyester sleeve catheters is larger in newly introduced dialysis patients.
