自制匀浆膳在慢性阻塞性肺疾病合并机械通气患者中应用效果评价

摘　　要：目的:探究慢阻肺加重患者机械通气过程中鼻饲自制匀浆膳的效果。方法:选取2016年1月~2018年12月某院80例慢阻肺疾病加重患者,数字随机表法分为两组各40例,均行常规对症治疗以及机械通气治疗,对照组行传统肠内营养剂鼻饲,观察组行自制匀浆膳鼻饲,对比两组生化指标、不良反应发生率、死亡率、医疗费用、脱机时间、ICU入住时间以及住院时间。结果:观察组血红蛋白(Hb)、外周血淋巴细胞(TCL)、血清总蛋白(TP)、血清白蛋白(ALB)和血清尿素氮(BUN)等生化指标改善均优于对照组(P<0.05);观察组患者不良反应发生率和死亡率显著低于对照组(P<0.05)。结论:慢阻肺患者机械通气中实施自制匀浆膳鼻饲能够有效改善患者生化指标,规避不良反应风险,减少死亡率,对患者的康复具有积极影响。Objective: To investigate the effect of nasal feeding self-made homogenate meal during mechanical ventilation in patients with chronic obstructive pulmonary aggravation. Methods: 80 patients with chronic obstructive pulmonary aggravation in a hospital from January 2016 to December 2018 were selected and divided into 2 groups with 40 patients in each group by digital random table method. Both were treated with routine symptomatic treatment and mechanical ventilation. The control group received traditional enteral nutrition nasal feeding, and the observation group performed self-made homogenized nasal feeding. Biochemical indicators, incidence of adverse reactions, mortality, medical costs, offline time, ICU stay and hospital stay were compared between the two groups. Results: The improvement of biochemical indexes such as Hb, TCL, TP, ALB and BUN in the observation group were better than those in the control group(P<0.05). The incidence of adverse reactions and mortality in the observation group were lower than those in the control group(P<0.05). Conclusion: Self-made homogenized nasal feeding in mechanical ventilation of patients with chronic obstructive pulmonary disease can effectively improve biochemical indicators,avoid adverse reactions and reduce mortality.
