摘 要:目的探讨虎符铜砭刮痧治疗糖尿病周围神经病变的临床疗效。方法选择2017年10月至2018年3月期间惠州市中医医院内分泌科收治的82例糖尿病周围神经病变患者为研究对象,根据随机数表法分为观察组和对照组,每组41例,对照组患者予甲钴胺治疗,观察组患者在此基础上应用虎符铜砭刮痧,疗程均为28 d。比较两组患者的临床疗效,治疗前后胫总及腓总神经的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)以及症状总评分法(TSS)评分的变化。结果观察组患者的治疗总有效率为85.37%,明显高于对照组的68.29%,差异有统计学意义(P<0.05);治疗后,观察组患者的TSS总分为(5.13±1.81)分,明显低于对照组的(7.68±1.26)分,差异有统计学意义(P<0.05);观察组患者的胫总神经、腓总神经的MNCV分别为(33.91±1.52) m/s、(33.69±1.41) m/s,SNCV分别为(33.97±1.44) m/s、(34.26±1.67) m/s,明显快于对照组的(31.98±1.61) m/s、(31.91±1.22) m/s、(32.02±1.38) m/s、(32.12±1.53) m/s,差异均有统计学意义(P<0.05)。结论虎符铜砭刮痧治疗糖尿病周围神经病变可以明显改善神经传导速度及患者的临床症状,提高临床疗效。Objective To investigate the clinical effect of Hufu Copper Gua Sha(scraping therapy) on diabetic peripheral neuropathy. Methods Eighty-two patients with diabetic peripheral neuropathy who were treated in Department of Endocrinology, Huizhou Traditional Chinese Medicine Hospital from October 2017 to March 2018 were selected as the subjects. They were divided into observation group and control group according to the random number table method, with 41 patients in each group. The control group was treated with mecobalamin, and the observation group was treated with Hufu copper Gua Sha. The treatment lasted for 28 days. The clinical efficacy, motor nerve conduction velocity(MNCV), sensory nerve conduction velocity(SNCV), and total symptom score(TSS) of tibial and peroneal nerves were compared before and after treatment. Results The total effective rate of patients in observation group was 85.37%, significantly higher than 68.29% of control group(P<0.05). After treatment, the total score of TSS in observation group was 5.13±1.81, significantly lower than 7.68±1.26 in control group(P<0.05). The MNCV of common tibial nerve and common peroneal nerve in observation group was(33.91 ± 1.52) m/s,(33.69 ± 1.41) m/s, and SNCV were(33.97 ± 1.44) m/s,(34.26 ± 1.67) m/s, significantly faster than(31.98 ± 1.61) m/s,(31.91 ± 1.22) m/s;(32.02 ± 1.38) m/s,(32.12 ± 1.53) m/s in the control group(P<0.05). Conclusion Hufu copper Gua Sha can significantly improve nerve conduction velocity and clinical symptoms of diabetic peripheral neuropathy, and improve clinical efficacy.
