丹参酮Ⅱ_A磺酸钠联合鼓室注射地塞米松治疗突发性耳聋临床评价

摘　　要：目的探讨丹参酮ⅡA磺酸钠结合鼓室注射地塞米松对突发性耳聋患者听力恢复程度、免疫功能、血液流变学的影响。方法选取医院2015年4月至2017年4月收治的突发性耳聋患者168例,随机分为联合组和对照组,各84例。对照组给予常规治疗和鼓室注射地塞米松治疗,联合组在对照组治疗基础上给予丹参酮ⅡA磺酸钠静脉滴注。结果治疗后,联合组耳鸣、耳闷、眩晕等听力恢复程度明显均优于对照组(P<0.05);两组T淋巴细胞亚群CD3^+和CD4^+及血清IgG和IgM水平均明显升高(P<0.05),联合组升高更显著(P<0.05);两组纯音听阈值及血液流变学指标全血高切黏度、全血低切黏度、血浆黏度、红细胞聚集指数均明显下降(P<0.05),且联合组均明显低于对照组(P<0.05);两组不良反应发生率比较无显著差异(P>0.05)。结论丹参酮ⅡA磺酸钠结合鼓室注射地塞米松治疗可有效提高突发性耳聋患者的听力水平,改善免疫功能和血液流变学指标,临床疗效显著。Objective To investigate the effect of sodium tanshinoneⅡA sulfonate combined with intratympanic injection of dexamethasone on hearing recovery,immune function and hemorheology in patients with sudden hearing loss.Methods Totally 168 patients with sudden hearing loss admitted to our hospital from April 2015 to April 2017 were selected and randomly divided into the combined group and the control group,84 cases in each group.The patients in the control group were given routine treatment and intratympanic injection of dexamethasone,while the patients in the combined group were given intravenous drip of sodium tanshinoneⅡA sulfonate on the basis of the control group.Results After treatment,the hearing recovery of tinnitus,aural fullness,and vertigo in the combined group were significantly better than that in the control group(P<0.05).The levels of T lymphocyte subsets CD3&+and CD4^+,and the levels of serum IgG and IgM in the two groups were significantly increased(P<0.05),and the increase in the combined group was more significant(P<0.05).The pure tone hearing threshold and hemorheological parameters of high whole blood viscosity,low whole blood viscosity,plasma viscosity and erythrocyte aggregation index in the two groups were significantly lower than those before treatment(P<0.05),and those in the combined group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Sodium tanshinoneⅡA sulfonate combined with tympanic injection of dexamethasone can effectively improve the hearing level of patients with sudden hearing loss,improve immune function and hemorheology parameters,which has significant clinical efficacy.
