吸入用复方异丙托溴铵溶液诱发震颤一例

摘　　要：1例80岁女性患者,诊断为"慢性支气管炎急性发作;右肺肺大泡;右肺肺炎;Ⅰ型呼吸衰竭",给予吸入用复方异丙托溴铵溶液、多索茶碱注射液、吸入用布地奈德混悬液及其他药物治疗;第2日,患者使用吸入用复方异丙托溴铵溶液10 min时,出现双手不自主震颤,立即停用该药,30 min后症状逐渐好转,后未再出现类似症状,病情稳定后出院。老年患者使用吸入用复方异丙托溴铵溶液时易发生震颤,应慎用并密切监测;尽量避免该药与吸入用布地奈德混悬液或多索茶碱注射液的联合应用,若须联合用药,应密切观察;应警惕有可能在用药第2日及以后发生震颤。One case of 80-year-old female patient was diagnosed with “acute attack of chronic bronchitis;bullae of right lung;pneumonia of right lung;type I respiratory failure”,who was given compound ipratropium bromide solution for inhalation combined with doxofylline injection,budesonide suspension for inhalation and other drugs;on the second day,the patient showed involuntary tremor of both hands at 10 min after taking compound ipratropium bromide solution for inhalation,then was given drug-withdrawal immediately;after 30 min,symptom of tremor was gradually improved and did not appear again,the patient was discharged after being stable.Elderly patients were more likely to be attacked by tremor when taking compound ipratropium bromide solution for inhalation,which should be used with caution and under close monitor;the combination application of compound ipratropium bromide solution for inhalation with inhaled budesonide suspension or doxofylline injection should be avoided,it should be closely monitored if drug combination was necessary;it should be alert to the possibility of tremor at the 2nd day and after of drug application.
