摘　　要：目的观察慢性活动期化脓性中耳炎咽鼓管功能障碍患者接受一期鼓室成形术治疗的疗效。方法以2015年1月-2018年2月在我院接受治疗的慢性活动期化脓性中耳炎咽鼓管功能障碍者(40例)为研究对象,对患者实施1年的随访,分析患者术后鼓膜愈合率及其听力恢复状况。结果 40例患者接受一期鼓室成形术治愈率为95.0%(38/40),这38例患者并没有发生鼓膜萎缩、钝角愈合等状况。有2例因鼓窦入口相对狭小,在早期就出现膜性闭合,缺少充足的时间使得咽鼓管功能迅速恢复,在3个月以后2例患者鼓膜再穿孔。研究发现,慢性活动期化脓性中耳炎咽鼓管功能障碍患者术前和术后气导听阀值存在明显差异,且患者术后骨气导差值比术前低,组间数据对比差异明显。结论慢性活动期化脓性中耳炎咽鼓管功能障碍者接受一期鼓室成形术治疗处理,并配合恰当的术后跟进治疗,就能获得理想的治疗效果。Objective To observe the curative effect of one-stage tympanoplasty on patients with eustachian tube dysfunction in chronic active suppurative otitis media. Methods Forty patients(40 cases) with eustachian tube dysfunction of chronic active suppurative otitis media treated in our hospital from January 2015 to February 2018 were taken as the research object, and the patients were followed up for one year to analyze the tympanic membrane healing rate and hearing recovery status after operation. Results The cure rate of 40 patients undergoing primary tympanoplasty was 95.0%(38/40). 38 patients did not suffer from tympanic membrane atrophy or obtuse angle healing. There were 2 cases with membranous closure in the early stage due to the relatively narrow entrance of tympanic antrum, and the eustachian tube function recovered rapidly due to lack of sufficient time. The tympanic membrane of 2 patients was re-perforated 3 months later. The study found that there were significant differences between preoperative and postoperative air conduction hearing thresholds in patients with chronic active suppurative otitis media with eustachian tube dysfunction, and the postoperative bone gas difference of patients was lower than that before surgery, with significant differences in data comparison between groups. Conclusion Patients with chronic active suppurative otitis media with eustachian tube dysfunction can receive onestage tympanoplasty treatment, and with appropriate follow-up treatment after operation, ideal therapeutic effect can be obtained.
