寻常型天疱疮累及食管黏膜

摘　　要：报告1例寻常型天疱疮累及食管黏膜。患者女,47岁。反复口腔黏膜糜烂伴呕血2年,复发3d。皮肤科检查:口腔上颚及牙龈糜烂及出血,尼氏征(+)。口腔黏膜组织病理检查:表皮角化过度,棘层松解,表皮内水疱,水疱内可见棘层松解细胞及红细胞。直接免疫荧光示复层鳞状上皮细胞间网状IgG(++)、C3(+)、IgA(-)及IgM(-)。间接免疫荧光:抗桥粒芯糖蛋白(Dsg)3抗体(+)及抗Dsg1抗体(-)。胃镜示食管黏膜剥脱出血。诊断:寻常型天疱疮累及食管黏膜。A case of pemphigus vulgaris involved esophageal mucosa is reported. A 47-year-old female presented with erosion on her oral mucosa with hematemesis for 2 years, and relapsed for 3 days. Dermatological examination showed erosion on the oral cavity palate and gum. Nikolsky sign was positive. Histopathological examination showed hyperkeratosis, acantholysis, intraepidermal blisters with acantholytic cells and red blood cells. Direct immunofluorescence showed reticular deposition of IgG and C3 between the stratified squamous epithelial cells, and negative for both IgA and IgM. Indirect immunofluorescence showed positive for anti-desmoglein(Dsg)3 and negative for anti-Dsg1. Gastroscopy revealed exfoliation and bleeding of esophageal mucosa. The diagnosis of pemphigus vulgaris involved esophageal mucosa was made.
