摘 要:目的探讨过氧化物酶体增殖物激活受体γ(PPARγ)基因C161T多态性与原发性高血压(EH)的关系。方法全面检索Pub Med、万方数据库、中国知网(CNKI)、维普资讯,收集关于PPARγ基因C161T多态性与EH发生关系的病例-对照研究,采用优势比(OR)和95%可信区间(95%CI)评估关联强度,应用Rev Man5. 3软件对纳入的研究进行异质性检验和效应值OR合并,漏斗图评估发表性偏倚。结果共检索到8篇文献,包括9个病例-对照研究,共累计EH患者1796例,对照组2162例。Meta分析结果显示:PPARγ基因C161T纯合子模型(TT vs CC)、杂合子模型(CT vs CC)、显性模型(TT+CT vs CC)、隐性模型(TT vs CC+CT)及T等位基因与EH的合并OR(95%CI)分别为0. 66(0. 49~0. 90)、0. 70(0. 52~0. 93)、0. 70(0. 52~0. 94)、0. 76(0. 56~1. 04)和0. 84(0. 64~1. 06),对应P值分别为0. 01、0. 01、0. 02、0. 08和0. 14。漏斗图未检测出显著的发表性偏倚。结论 PPARγ基因C161T多态性与EH易感性相关。Objective To evaluate the relationship between peroxisome proliferator-activated receptor γ( PPARγ) C161 T genetic polymorphisms and the risk of essential hypertension( EH). Methods Relevant literatures were extensively searched in Pub Med,Wanfang Databases,CNKI and VIP for collecting the case-control studies investigating the relationship between PPARγC161 T genetic polymorphisms and EH. Odds ratios( OR) with 95% confidence intervals( CI) were applied to assess the strength of association. The Rev Man 5. 3 software was applied for heterogeneity test and combined OR and their 95% CIs calculation. Publication bias was assessed through funnel plot. Results Eight related literatures( Nine case-control studies)on the relationship between PPARγC161 T genetic polymorphisms and EH were included,involving 1796 patients( casegroups) and 2162 controls. The combined OR( 95% CI) for PPARγ gene C161 T Homozygous model( TT vs CC),Heterozygote model CT vs CC),dominant model( TT + CT vs CC),Recessive model( TT vs CC + CT) and T allele were 0. 66( 0. 49 ~0. 90),0. 70( 0. 52 ~ 0. 93),0. 70( 0. 52 ~ 0. 94),0. 76( 0. 56 ~ 1. 04) and 0. 84( 0. 64 ~ 1. 06),respectively with P =0. 01,0. 01,0. 02,0. 08 and 0. 14. No significant publication bias was detected by funnel plot. Conclusions The C161 T genetic polymorphism of PPARγ was associated with the susceptibility of EH.
