摘　　要：目的研究以前列地尔为主,并控制血压、血糖、血脂,多因素干预Ⅲ期糖尿病肾病的疗效。方法收集2015年1月至2017年12月于我院门诊、住院治疗,诊断为Ⅲ期糖尿病肾病的患者93例,将患者按是否应用前列地尔分为两组,分别为:观察组(应用前列地尔)45例;安慰剂对照组(未应用前列地尔)48例。在干预患者活动量,优质低蛋白质饮食的基础上,控制患者血压、血糖、血脂,分别统计观察组与对照组不同时间段肾小球滤过率(内生肌酐清除率Ccr)、24 h尿蛋白量(U-TP)指标。结果观察组患者疗效优于对照组患者,延长治疗时间,疗效更为明显(P <0.05),血脂亦明显减低;血压、血糖、血脂控制达标的患者疗效优于血压、血糖、血脂未达标的患者。结论以前列地尔为主,多因素干预Ⅲ期糖尿病肾病有一定疗效,坚持长期治疗效果更佳,值得临床应用。Objective To understand the efficacy of previous treatment by the alprostadil combination multi-factor intervention,including control blood pressure,blood sugar and blood lipids,for stage Ⅲ diabetic nephropathy. Methods 93 patients with stageⅢ diabetic nephropathy were enrolled in our hospital from January2015 to December 2017. The patients were divided into two groups according to the application of alprostadil:45 patients were alprostadil group;48 patients were Placebo control group(pro-lindil). On the basis of intervention of the patient′s physical activity level andhigh-quality low-protein diet,control the patient′s blood pressure,blood sugar,and blood lipids.Statistics of glomerular filtration rate(endogenous creatinine clearance Ccr)and 24-hour urine Protein amount(U-TP)indicator were calculated in the two groups. Results The patients in the alprostadil group were better than the Placebo group. Prolonged the treatment time,and the curative effect was more obvious(P < 0.05). The blood lipids were also significantly reduced. The patients with standard blood pressure,blood sugar and blood lipid have better effect. Conclusion In short,alprostadil combination multi-factor intervention on stage Ⅲ diabetic nephropathy has certain curative effect. It is worthy for long-term treatment and clinical application.
