摘  要:目的:总结磁共振扩散加权成像及动态增强扫描在肝脏占位性病变中的应用价值。方法:收集2017年9月-2018年6月中山大学肿瘤防治中心确诊的40例(55个病灶)肝脏占位病变患者临床资料,均接受MRI平扫及动态增强扫描、DWI扫描联合动态增强扫描,比较两种方式的诊断效果。结果:两种方式诊断肝囊肿均为100%,DWI扫描联合动态增强扫描诊断肝脏海绵状血管瘤、肝细胞肝癌、肝脏转移瘤准确率均明显高于MRI平扫及动态增强扫描,差异存在统计学意义(P<0.05)。结论:DWI应用肝脏占位性病变的诊断中确有较高敏感性与特异性,通过与动态增强扫描联合检查,有助于明确肝脏肿瘤类型,增强诊断信息,为临床治疗提供依据。Objective: To summarize the application value of MR diffusion-weighted imaging(DWI) and dynamic contrast enhancement(DCE) in hepatic space-occupying lesions. Methods: The clinical data of 40 patients(55 lesions) with hepatic space-occupying lesions diagnosed by the Sun Yat-sen University Cancer Center from September 2017 to June 2018 were collected. All patients underwent MRI plain and DCE, DWI combined with DCE, and compared the diagnostic effects of the two methods. Results: The diagnosis of hepatic cysts were 100% in both ways. The accuracy of DWI scanning combined with DCE in the diagnosis of hepatic cavernous hemangioma, hepatocellular carcinoma and hepatic metastases were significantly higher than that of MRI plain and DCE(P<0.05). Conclusion: DWI has a high sensitivity and specificity in the diagnosis of hepatic space-occupying lesions. It is helpful to identify the type of liver tumors, enhance the diagnostic information, and provide evidence for clinical treatment through dynamic enhanced scanning combined examination.
