熄风通络汤联合西药治疗对高血压患者血管内皮功能的影响

摘　　要：目的:探讨熄风通络汤联合西药治疗对原发性高血压(EH)患者血管内皮功能的影响。方法:遴选184例EH患者随机分为对照组(n=92)和观察组(n=92),对照组给予常规西药治疗,观察组在以上西医治疗基础上给予熄风通络汤治疗,观察两组治疗前及治疗28d后的血压、中医症状积分,临床疗效,治疗前及治疗28d后的血管内皮功能及血脂指标变化。结果:治疗28d后,两组舒张压(DBP)、收缩压(SBP)值显著下降(P<0.05),中医症状积分显著减少(P<0.05),观察组变化幅度显著大于对照组(P<0.05)。组间对比临床总有效率发现,观察组(92.39%)显著高于对照组(80.43%)(P<0.05)。治疗28d后,两组血清内皮素-1(ET-1)、超敏C反应蛋白(hs-CRP)水平显著降低(P<0.05),一氧化氮(NO)水平显著升高(P<0.05),观察组变化幅度显著大于对照组(P<0.05)。治疗28d后,两组血清三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)显著降低,高密度脂蛋白胆固醇(HDL-C)水平显著升高(P<0.05),观察组以上血脂指标除HDL-C之外的变化幅度显著大于对照组(P<0.05)。结论:熄风通络汤联合西药治疗EH患者疗效确切,可改善患者血压水平和相关症状,提高血管内皮舒张功能,有效调节血脂水平。Objective:To explore the effects of Xifeng Tongluo decoction combined with western medicine on vascular endothelial function in patients with essential hypertension(EH).Methods:A total of 184 patients with EH were randomly divided into control group(n=92)and observation group(n=92).Control group was treated with conventional western medicine,and observation group was treated with Xifeng Tongluo decoction on the basis of above western medicine treatment.Blood pressure and TCM symptoms scores before treatment and after 28 d of treatment,clinical efficacy,vascular endothelial function and blood lipids indexes before treatment and after 28 d of treatment were observed in the two groups.Results:After 28 d of treatment,the values of diastolic blood pressure(DBP)and systolic blood pressure(SBP)were significantly decreased(P<0.05),and the scores of TCM symptoms were significantly decreased(P<0.05),and the changes in observation group were significantly greater than those in control group(P<0.05).The total clinical effective rate in the observation group was significantly higher than that in control group(92.39%vs 80.43%)(P<0.05).After 28 d of treatment,serum levels of endothelin-1(ET-1)and high-sensitivity C-reactive protein(hs-CRP)were significantly decreased(P<0.05)while the nitric oxide(NO)level was significantly increased(P<0.05),and the changes in observation group were significantly greater than those in control group(P<0.05).After 28 d of treatment,the levels of serum triglyceride(TG),total cholesterol(TC)and low-density lipoprotein cholesterol(LDL-C)were significantly decreased while the high-density lipoprotein cholesterol(HDL-C)level was significantly increased(P<0.05),and the changes of blood lipid indexes except HDL-C in observation group were significantly larger than those in control group(P<0.05).Conclusion:Xifeng Tongluo decoction combined with western medicine has exact efficacy in treating EH patients,and it can improve blood pressure level and related symptoms,enhance vascular endothelial dilation func
