摘 要:目的分析血浆B型脑钠肽在评估慢性阻塞性肺疾病病情中的临床价值。方法选择我院2014年1月至2018年9月收治的124例慢性阻塞性肺疾病患者,根据患者病情分为稳定组71例和急性加重组53例,选择同期健康者30例作为对照组,比较三组的BNP水平、肺功能和血气分析指标,分析慢性阻塞性肺疾病患者血浆BNP水平与肺功能及血气分析指标的相关性。结果急性加重组的血浆BNP水平显著高于稳定组和对照组,FEV1、FEV1/FVC、PaO2值均显著低于稳定组和对照组,PaCO2值显著高于稳定组和对照组,差异均有统计学意义(P<0.05)。慢性阻塞性肺疾病患者的血浆BNP水平与PaO2、FEV1%、FEV1/FVC呈负相关(P<0.05),与PaCO2水平呈正相关(P<0.05)。结论血浆BNP水平越高,慢性阻塞性肺疾病病情越严重,BNP水平可随患者肺功能和血气分析指标的改善而降低。Objective To analyze the clinical value of plasma B-type natriuretic peptide(BNP) in the evaluation of condition of chronic obstructive pulmonary disease(COPD). Methods From January 2014 to September 2018, 124 cases of COPD patients admitted to our hospital were selected and divided into the stable group(n = 71) and the acute exacerbation group(n = 53) according to the disease condition;30 healthy subjects at the same period were selected as the control group. The levels of BNP, and indictors of lung function and blood gas analysis were compared among the three groups, and the relationships between BNP level and indicators of lung function and blood gas analysis were analyzed. Results The plasma BNP level of the acute exacerbation group was significantly higher than that of the stable group and the control group, the value of FEV1, FEV1/FVC and PaO2 were significantly lower than those of the stable group and the control group, and the value of PaCO2 was significantly higher than that of the stable group and the control group(P <0.05). The plasma BNP level of COPD patients was negatively correlated with PaO2, FEV1% and FEV1/FVC, and positively correlated with PaCO2(P <0.05).Conclusions The higher the plasma BNP level, the more serious the condition of chronic obstructive pulmonary disease, and the BNP level can be reduced with the improvement of lung function and blood gas analysis indicators.
