摘　　要：目的:分析比较治疗轻度精索静脉曲张(VC)合并精液异常两种方式的临床疗效。方式:回顾性总结分析2014年3月1日至2018年12月1日期间于佛山市妇幼保健院生殖中心诊断为轻度VC且合并精液质量异常患者307例,对比两种方式治疗前后的临床疗效。其中通过前列通瘀胶囊联合生精胶囊治疗组(中医药组)178例,显微镜下精索静脉结扎术治疗组(手术组)129例。统计两组患者治疗前及治疗后3个月的精液量、精子总数、前向运动精子百分率(PR)、精子畸形率、精子DNA碎片指数(DFI)临床指标。结果:两组患者治疗前后各项指标除了精液量之外,差异均具有统计学意义(P <0.05),与中医药组相比,手术组患者精子总数、PR、精子畸形率、DFI的改善更为明显,差异均具有统计学意义(P <0.05)。结论:与中医药治疗相比,显微镜精索静脉结扎术可更为有效地改善轻度VC合并精液质量异常患者的精液质量,从而提高患者生育能力。Objective To investigate the clinical effect of two treatment methods for mild varicocele combined with abnormal semen. Methods A retrospective analysis was conducted on 307 patients diagnosed with mild varicocele and abnormal semen quality from March 1 st, 2014 to December 1 st, 2018, and the clinical efficacy before and after treatment with two different methods was compared. Among them, 178 cases were treated by qianlietongyu capsule combined with shengjing capsule(traditional Chinese medicine treatment group), and 129 cases were treated by microscopically ligated spermatic vein(operation treatment group). The semen volume, total number of sperm, percentage of forward moving sperm(PR), sperm malformation rate, sperm DNA fragment rate and other clinical indicators of the two groups before and 3 months after treatment were calculated. Results The indexes of the two groups were significantly different except for the amount of semen before and after treatment. Compared with the Chinese medicine group, the total sperm count, the percentage of sperm in the forward movement(PR), the rate of sperm deformity, and the rate of sperm DNA fragmentation in the surgical group. The improvement of the indicators was more obvious, and the difference was statistically significant(P < 0.05). Conclusion Compared with traditional Chinese medicine, microscopical spermatic vein ligation can improve the semen quality of patients with mild varicocele and abnormal semen quality more effectively, thus improving the fertility of patients.
