经后腹膜和经腹腔手术入路行腹腔镜肾上腺手术的效果比较

摘　　要：目的比较经后腹膜和经腹腔手术入路行腹腔镜肾上腺手术的效果。方法80例腹腔镜肾上腺手术患者根据手术入路的不同分为经后腹膜组和经腹腔组,每组40例。其中巨大肾上腺肿瘤(>6cm)经后腹膜组10例,经腹腔组13例。比较两种入路的手术时间、术中出血量、术后住院时间、术后并发症发生率等。结果经后腹膜组和经腹腔组的手术时间、术中出血量、术后住院时间、术后并发症发生率差异无统计学意义(P>0.05)。在巨大肾上腺肿瘤手术中,经后腹膜入路的手术时间、术中失血量多于经腹腔入路(P<0.01),而两组的术后住院时间、术后并发症发生率差异无统计学意义(P>0.05)。结论腹腔镜经后腹膜入路和经腹腔入路在肾上腺手术总体效果无差异,但对于巨大肾上腺肿瘤,经腹腔入路可能更具优势。Objective To compare the effect of transretroperitoneal and transabdominal laparoscopic adrenalectomy. Methods A total of 80 patients receiving laparoscopic adrenalectomy were divided into the Transretroperitoneal Group and Transabdominal Group according to the different operative approaches, 40 cases in each group. There were 23 cases of giant adrenal tumors (>6 cm), of which 10 cases received transretroperitoneal laparoscopic adrenalectomy and 13 cases received transabdominal laparoscopic adrenalectomy. The two groups were compared in terms of operation duration, intraoperative bleeding, postoperative hospital stay and incidence of postoperative complications. Results There was no statistical difference in the operation duration, intraoperative bleeding, postoperative hospital stay and incidence of postoperative complications between the Transretroperitoneal Group and Transabdominal Group (P>0.05). In the operation for giant adrenal tumors, the Transretroperitoneal Group had the operation duration and intraoperative bleeding greater than the Transabdominal Group (P<0.01). There was no statistical difference in the postoperative hospital stay and the incidence of postoperative complications between the two groups (P>0.05). Conclusion There is no difference in overall effect of transretroperitoneal and transabdominal laparoscopic adrenalectomy. However, for giant adrenal tumors, transabdominal laparoscopic adrenalectomy may be more advantageous.
