摘　　要：目的:探讨PHENIX-8PLUS仿生物电刺激治疗外阴上皮非瘤样病变(NNEDV)效果及作用机制。方法:选取2017年1月至2018年1月在广东省东莞市人民医院就诊的NNEDV患者73例,按随机数字表法分为观察组(n=38)和对照组(n=35)。对照组给予丙酸睾丸酮治疗,观察组给予PHENIX-8PLUS仿生物电刺激治疗。比较两组临床疗效、临床症状和体征评分以及治疗前、后血管内皮生长因子(VEGF)表达及微血管密度(MDV),并记录随访6个月复发率。结果:观察组中,外阴慢性单纯性苔藓(LSC)和外阴硬化性苔藓(LSV)患者治疗总有效率均为100%,显著高于对照组的63.16%、68.75%(均P<0.05)。观察组LSC和LSV患者疗程结束及随访3个、6个月时,瘙痒、疼痛、皮肤弹性、皮肤颜色、病变范围评分均低于对照组(均P<0.05)。观察组疗效结束LSC、LSV患者VEGF阳性表达率和MDV值显著高于对照组(P<0.05),复发率低于对照组(P<0.05)。结论:PHENIX-8PLUS仿生物电刺激治疗NNEDV可持续缓解临床症状,提高临床疗效,机制可能与提高VEGF的表达和MDV有关。Objective: To investigate the effect and mechanism of PHENIX-8 PLUS bionic electrical stimulation on nonneoplastic epithelial disorders of vulva (NNEDV). Methods: A total of 73 patients with NNEDV who were admitted to our hospital from January 2017 to January 2018 were selected,and randomly divided into an observation group ( n =38) and a control group ( n =35).The patients in the control group were treated with testosterone propionate,and those in the observation group were treated with PHENIX-8 PLUS bionic electrical stimulation.The clinical curative effect,clinical symptom scores and recurrence rate were compared between the two groups.The vascular endothelial growth factor (VEGF) expression and microvessel density (MDV) value were detected. Results: The response rates of lichen simplex chronicus(LSC) and lichen sclerosus of vulva (LSV) in the observation group were higher than those in the control group ( P <0.05).The scores of itching,pain,skin elasticity,skin color and range of lesions in patients with LSC and LSV in the observation group at the end of treatment and after 3 and 6 months of follow-up were lower than those in the control group ( P <0.05).The positive expression rate of VEGF and MDV in patients with LSC and LSV in the observation group were significantly higher than those in the control group ( P <0.05).The recurrence rate in the observation group was lower than that in the control group ( P <0.05). Conclusion: PHENIX-8 PLUS bionic electrical stimulation for NNEDV could effectively relieve clinical symptoms and improve clinical curative effect.The mechanism may be related to the increase of local MDV.
