甘精胰岛素注射液联合阿卡波糖片治疗磺脲类继发性失效的2型糖尿病患者的疗效分析

摘　　要：目的探讨甘精胰岛素联合阿卡波糖对磺脲类降糖药继发性失效的2型糖尿病的疗效。方法选取2018年1月~2018年12月收治的磺脲类降糖药继发性失效的2型糖尿病患者180例为研究对象,将患者随机分为对照组和观察组,每组90例。两组患者均停用原药物治疗方案,对照组患者予甘精胰岛素注射液治疗,研究组患者予以甘精胰岛素注射液联合阿卡波糖治疗,比较两组患者治疗前后血糖水平及糖化血红蛋白水平和两组患者胰岛素用量。结果治疗前两组患者血糖水平及糖化血红蛋白水平无统计学差异;治疗后,研究组的空腹血糖水平、餐后2h血糖水平、糖化血红蛋白水平较对照组显著降低,差异有统计学意义(P<0.05),研究组患者的甘精胰岛素用量较对照组用量减少,差异有统计学意义(P<0.05)。结论甘精胰岛素联合阿卡波糖治疗磺脲类降糖药继发性失效的2型糖尿病,可显著降低患者空腹血糖水平、餐后2h血糖水平、糖化血红蛋白水平,同时联合用药可减少甘精胰岛素用量。Objective To investigate the clinical effect of insulin glargine combined with acarbose in type 2 diabetes with secondary failure of sulfonylureas. Methods A total of 180 type 2 diabetes patients with secondary sulfonylurea failure that admitted in 2018 were divided into the control group and the observation group randomly. The treatment with insulin glargine and insulin glargine combined with acarbose were given to observation group and control group separately. Then the differences between the two groups were compared. Results The level of blood sugar and HbA1c in patients of observation group decreased significantly( P < 0.05). Meanwhile, the dosage of insulin glargine in the observation group is lower than that of the control group( P < 0.05). Conclusion Insulin glargine combined with acarbose in the treatment of type 2 diabetes with secondary failure of sulfonylurea hypoglycemic drugs can significantly reduce the level of fasting blood glucose, 2-hour postprandial blood glucose and glycosylated hemoglobin. At the same time, combined use of insulin glargine can reduce the dosage of insulin glargine, which has high clinical value and is worth promoting.
