桂枝汤联合多潘立酮治疗小儿功能性消化不良(脾胃虚弱)随机平行对照研究

摘　　要：[目的]观察桂枝汤联合多潘立酮治疗小儿功能性消化不良(脾胃虚弱)疗效。[方法]使用随机平行对照方法,将60例门诊患儿按就诊顺序号抽签随机分为两组。对照组30例多潘立酮,10mg/次,3次/d,饭前15~20min服用。治疗组30例桂枝汤(桂枝、芍药、姜生各9g,麦芽、神曲、鸡内金各10g,大枣3颗,甘草6g),水煎50mL,1剂/d,2次/d;实热积滞、腹满实痛加大黄6g,无苔加女贞子6g,服药时辅以热粥,提升药力;多潘立酮治疗同对照组。连续治疗7d为1疗程。观测临床现、症状评分、症状消失时间(食欲不振、恶心呕吐、反酸嗳气、腹痛腹胀)、不良反应。治疗1疗程(7d),判定疗效。[结果]治疗组痊愈24例,显效5例,有效1例,无效0例,总有效率100.00%;对照组痊愈8例,有效6例,有效10例,无效6例,总有效率80.00%;治疗组疗效优于对照组(P<0.05)。症状评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。症状消失时间治疗组短于对照组(P<0.01)。[结论]桂枝汤联合多潘立酮治疗小儿功能性消化不良(脾胃虚弱),疗效满意,无严重不良反应,值得推广。[Objective] To observe the curative effect of Guizhi decoction combined with domperidone on children with functional dyspepsia(spleen and stomach weakness). [Method] 60 out-patients were randomly divided into two groups by drawing lots according to the order of visiting. In the control group, 30 patients received domperidone 10 mg/time, 3 times/day, 15~20 minutes before meals. In the treatment group, 30 cases of Guizhi decoction(Guizhi, Shaoyao, Shengjiang each 9 g, Maiya, Shenqu, Jineijin each 10 g, Dazao3, Gancao 6 g) were decocted in water for 50 mL, 1 dose per day, 2 times per day;the accumulation of excess heat, excessive abdominal pain increased by Dahuang 6 g, no moss plus Nvzhenzi 6 g, supplemented with hot porridge to enhance the medicinal power;domperidone treatment was the same as the control group. Continuous treatment for 7 days is a course of treatment. Clinical manifestations, symptom scores,disappearance time of symptoms(anorexia, nausea and vomiting, acid regurgitation, abdominal pain and abdominal distension) and adverse reactions were observed. One course of treatment(7 days) was used to determine the curative effect. [Result] In the treatment group, 24 cases were cured, 5 cases were markedly effective, 1 case was effective, 0 cases were ineffective, and the total effective rate was 100.00%. In the control group, 8 cases were cured, 6 cases were effective, 10 cases were effective and 6 cases were ineffective,the total effective rate was 80.00%. The curative effect of the treatment group was better than that of the control group(P<0.05). Symptom scores were improved in both groups(P<0.01), and the improvement in treatment group was better than that in control group(P<0.01). The disappearance time of symptoms in the treatment group was shorter than that in the control group(P<0.01). [Conclusion] Guizhi decoction combined with domperidone in the treatment of children with functional dyspepsia(spleen and stomach weakness) has satisfactory curative effect and no serious adverse reactions, which is w
