体质辨识穴位贴敷联合西药治疗支气管哮喘随机平行对照研究

摘　　要：[目的]观察体质辨识穴位贴敷联合西药治疗支气管哮喘疗效。[方法]使用随机平行对照方法,将90例门诊患者按就诊顺序号抽签随机分为两组。对照组45例布地奈德2mg+4mL生理盐水雾化吸入,8h 1次;舒利迭100~200μg/次,2次/d。治疗组45例体质辨识贴敷,取穴:天突、大椎、膻中、中府、膏肓、肺俞、脾俞、肾俞;阳虚质-扶阳定喘膏,附子熟、巴戟天、补骨脂、麻黄、肉桂、白芥子、百部各5g;气虚质-玉屏风膏:黄芪、白术、防风,山茱萸、地黄熟、山药、杏仁、茯苓、甘草炙各5g,研成细末,生姜汁调和成药丸,取一次性医用贴敷贴固定于穴位处,4~6h/穴·次-1,2次/周;西药治疗同对照组。连续治疗4周为1疗程。观测临床表现、FEV1、FEV1/FVC、PEF、、不良反应。治疗1疗程(4周),判定疗效。[结果]治疗组临床控制25例,显效11例,有效7例,无效2例,总有效率95.56%;对照组临床控制20例,显效9例,有效9例,无效9例,总有效率80.00%;治疗组疗效优于对照组(P<0.05)。FEV1、FEV1/FVC、PEF、两组均有改善(P<0.05,P<0.01),治疗组改善优于对照组(P<0.05)。[结论]体质辨识穴位贴敷联合西药治疗支气管哮喘,疗效满意,无严重不良反应,值得推广。[Objective] To observe the therapeutic effect of acupoint application of constitution identification combined with western medicine on bronchial asthma. [Method] Ninety outpatients were randomly divided into two groups by drawing lots according to the order of visiting. The control group consisted of 45 patients inhaled budesonide 2 mg+4 mL saline for 8 hours once, and Seretide 100-200 ug/time, twice a day. In the treatment group, 45 cases were treated by physical identification and application, and acupoints were selected:Tiantuo, Dazhui, Yinzhong, Zhongfu, Gaokui, Feishu, Pishu and Shenshu;Yangxu-Fuyang Dingchuan ointment, Shufuzi, Bajitian, Buguzhi, Mahuang, Rougui, Baijiezi, Baibu each 5 g;Qixu-Yupingfeng ointment:Huangqi, Baizhu, Fangfeng, Shanzhuyu, Shudihuang, Shanyao, Xingren, Fuling, Zhigancao 5 g, respectively.Fine powder, ginger juice mixed into pills, take appropriate amount to one-time medical application fixed at the acupoint, 4-6 h/acupoint·times-1, 2 times/week;western medicine treatment with the control group.Continuous treatment for 4 weeks is a course of treatment. Clinical manifestations, FEV1, FEV1/FVC, PEF,and adverse reactions were observed. One course of treatment(4 weeks) was used to determine the curative effect. [Result] In the treatment group, 25 cases were clinically controlled, 11 cases were markedly effective,7 cases were effective, 2 cases were ineffective, and the total effective rate was 95.56%. In the control group,20 cases were clinically controlled, 9 cases were markedly effective, 9 cases were effective and 9 cases were ineffective, the total effective rate was 80.00%. The curative effect of the treatment group was better than that of the control group(P<0.05). FEV1, FEV1/FVC, PEF and two groups all improved(P<0.05, P<0.01), and the improvement of treatment group was better than that of control group(P<0.05).
