不同时辰复方藤酱汤保留灌肠在湿热瘀结型慢性盆腔炎病人护理中的应用

摘　　要：[目的]观察复方藤酱汤不同时辰保留灌肠对湿热瘀结型慢性盆腔炎病人的护理效果。[方法]将90例湿热瘀结型慢性盆腔炎病人按照信封法分为A组(卯时组)、B组(辰时组)、C组(酋时组)各30例,在常规治疗和护理的基础上,3组病人分别在卯时、辰时、酋时进行复方藤酱汤保留灌肠,干预时间为2周,观察3组病人的护理效果。[结果]A组护理后总有效率为100.0%,高于B组(90.0%)和C组(86.7%);A组肿瘤坏死因子(TNF-α)低于B组、C组,绿原酸、齐墩果酸浓度高于B组、C组,3组比较差异均有统计学意义(P<0.05)。[结论]湿热瘀结型慢性盆腔炎的病人在大肠经气血流最旺盛时(卯时)选用归经的复方藤酱汤保留灌肠,可促进中药汤剂有效成分通过肠道黏膜吸收,发挥药物的最佳药效。Objective: To observe the nursing effects of retention enema with compound rattan sauce soup at different times in the nursing of patients with chronic pelvic inflammatory disease caused by stasis of heat and dampness. Methods: According to the envelope method,90 cases of chronic pelvic inflammatory disease patients that belonged to the type of stasis of heat and dampness were divided into group A (the period of the day from 05:00-07:00),group B (the period of the day from 07:00-09:00) and group C (the period of the day from 17:00-19:00),30 cases in each group.On the basis of routine treatment and nursing,three groups of patients had preserved enema with compound rattan sauce soup at different times of the day for 2 weeks.And the nursing effects of three groups of patients were observed. Results: The total effective rate of group A was 100.0%,was higher than that of group B (90.0%) and group C (86.7%).The tumor necrosis factor(TNF-α) in group A were lower than those in group B and C,and the concentrations of chlorogenic acid and oleanolic acid were higher than those in group B and C.The differences between the three groups were statistically significant ( P <0.05). Conclusions: When the circulation of large intestine is the most vigorous (the period of the day from 05:00-07:00),patients with chronic pelvic inflammatory disease caused by the stasis of heat and dampness have retention enema with compound rattan sauce soup,which can promote the effective absorption of the effective ingredients of traditional Chinese medicine decoction through intestinal mucosa,and maximize the effectiveness of medicine.
