摘　　要：目的:评价万古霉素血药浓度监测在我院的临床应用情况,分析血药浓度监测对不良反应发生率、治疗有效率、用药天数及肾功能的影响。方法:建立HPLC法监测万古霉素血药浓度的方法学,抽取我院2016年4月~2017年10月进行万古霉素血药浓度监测(HPLC法)的38例患者,对监测数据、用药天数、有效率及监测前后肾功能变化情况进行对照分析。结果:38例患者的57次监测结果显示:谷浓度在10~20μg·mL^-1的占33.9%;高于20μg·mL^-1的占7.14%;5~10μg·mL^-1范围内占46.46%;低于5μg·mL^-1的占12.5%。血药浓度监测对使用万古霉素的治疗有效率、不良反应发生率都有显著影响,肾功能与监测万古霉素血药浓度前比较差异有统计学意义(P<0.05)。结论:根据万古霉素血药浓度监测值制定个体化用药方案可以提高万古霉素治疗有效率并减少不良反应,从而促进万古霉素合理用药。Objective: To evaluation the clinical application of vancomycin blood concentration monitoring in our hospital patients,to analyze of the effect of blood drug concentration monitoring on the incidence rate of adverse reactions,treatment efficiency ,medication duration and renal function. Methods:First,to establish a HPLC method for vancomycin blood concentration monitoring. Then select 38 patients who were monitored for vancomycin blood concentration in our hospital from April 2016 to October 2017.Last analysis the monitoring data,medication duration, treatment efficiency and changes in renal function before and after monitoring. Results:57 monitoring results of 38 patients showed: The concentration of the valley is 33.9% at 10~20μg·mL^-1, 7.14% above 20μg·mL^-1,46.46% at 5~10μg·mL^-1 12.5%blow 5μg·mL-1Blood concentration monitoring has a significant effect on the treatment efficiency and adverse reaction rate of patients who use vancomycin. The difference between renal function and pre- monitoring is statistically significant. Conclusion:According to the detection value of vancomycin blood concentration, the individualized medication plan for patients can effectively improve the treatment efficiency of vancomycin, and reduce the occurrence of adverse reactions, thereby promoting the rational use of vancomycin.
