探讨反复输血患者IL及可溶性IL-2受体与输血不良反应的关系

摘　　要：目的分析反复输血患者IL及可溶性IL-2受体水平与输血不良反应关联。方法选取该院2018年收治的200例反复输血患者,于输血前后空腹采集静脉血进行IL及可溶性IL-2受体检测,分析其与输血不良反应的关联。结果输血后1、3个月,患者的IL及可溶性IL-2受体水平均较输血前有明显降低,差异有统计学意义(P<0.05);输血后3个月的IL及可溶性IL-2受体水平均较输血后1个月有明显提升,差异有统计学意义(P<0.05)。200例患者输血治疗期间有13例出现不良反应,发生率为6.5%,其中包括非溶血性发热反应6例(3.0%),过敏反应3例(1.5%),溶血反应1例(0.5%),继发感染2例(1.0%),其他1例(0.5%)。输血前与输血后1、3个月的IL及可溶性IL-2受体水平下降与输血不良反应的关联均呈正相关性(P<0.05)。结论长期反复输血患者的不良反应发生率较高,且输注后患者的IL及可溶性IL-2受体水平会有一定程度的下降,二者之间的关联呈正相关性。Objective To analyze the correlation between the levels of IL and solubl IL-2 receptor and adverse reactions of blood transfusion in patients with repeated transfusion. Methods 200 cases of repeated blood transfusion patients admitted to the hospital in 2018 were selected and fasting venous blood was collected before and after transfusion to detect IL and soluble IL-2 receptor,and to analyze the relationship between IL-2 receptor and adverse reactions of blood transfusion. Results The levels of IL and soluble IL-2 receptor decreased significantly in 1 and 3 months after transfusion compared with those before transfusion( P <0.05),and the levels of IL and soluble IL-2 receptor increased significantly in 3 months after transfusion compared with that in 1 month after transfusion( P <0.05).During the period of blood transfusion treatment,13 patients had adverse reactions (6.5%) including non-hemolytic febrile reactions in 6 cases (3.0%),allergic reactions in 3 cases (1.5%),hemolytic reactions in 1 case (0.5%),secondary infections in 2 cases (1.0%) and other 1 case (0.5%).There was a positive correlation between the decrease of IL and soluble IL-2 receptor levels before and 1 and 3 months after transfusion and the adverse effects of transfusion( P <0.05). Conclusion The incidence of adverse reactions in patients with long-term repeated transfusion is high,and the levels of IL and soluble IL-2 receptor will decrease to some extent after transfusion,and the correlation between them is positive.
