摘  要:目的探讨体质指数(BMI)与伴有慢性肺部疾病的心房颤动患者的心房顺应性的关系。方法回顾性分析2013年1月至2014年12月首都医科大学附属北京安贞医院和北京同仁医院87例伴有慢性肺部疾病的心房颤动患者的BMI和心房顺应性。根据BMI,将患者分为4组:偏轻3例,BMI<18.5kg/m^2;正常18例,BMI:18.5~23.9kg/m^2;偏重42例,BMI:24~27.9kg/m^2;肥胖24例,BMI≥28kg/m^2。分别采用箱式图和加权回归的方法探讨BMI与心房顺应性的关系。结果87例伴有慢性肺部疾病的心房颤动患者中,BMI与心房容量指数呈“U”型关系。BMI正常时,左房容量指数最低(20.86±1.94);BMI偏重时,右心房容量指数为最低(19.71±1.29)。左、右心房峰值纵应变随着BMI增加而降低,BMI偏轻时,左、右心房峰值纵应变最高,而BMI肥胖时,左、右心房峰值纵应变最低。结论伴有慢性肺部疾病的心房颤动患者的BMI与心房容量指数呈“U”型关系,与峰值纵应变呈近线性关系。伴有慢性肺部疾病的肥胖心房颤动患者的心房顺应性较低。Objective To explore the relationship between body mass index (BMI) and atrial compliance in patients with atrial fibrillation (AF) and chronic lung disease (CLDs). Methods Eighty-seven patients with AF and CLDs were retrospectively analyzed in Beijing Anzhen Hospital and Beijing Tongren Hospital from 2013 to 2014. The patients were divided into four groups according to their BMI, including under weight (BMI<18.5);normal weight (BMI: 18.5-23.9);overweight (BMI: 24-27.9);and obese (BMI≥28). Association of BMI with atrial compliance was analyzed using Box Plot, and weighted regression method with BMI as a continuous variable. Results In 87 patients with AF and CLDs, there was a significant correlation between BMI and atrial volume index demonstrating "U" shape. The left atrial volume index was lowest (20.86±1.94) with normal weight patients. The right atrial volume index was lowest in overweight patients (19.71±1.29). Both the left and right atrial peak longitudinal strain decreased with increased BMI. Conclusions With the increase of BMI, both the left and right atrial compliance shows a decreasing trend in AF patients with CLD. The obese patients with AF and CLDs had relatively low atrial compliance.
