山东某三甲医院临床抗菌药物使用情况分析

摘　　要：目的实地调研山东省某三甲医院临床抗菌药物使用现状,指导临床合理使用抗菌药物。方法采用实地回顾性调查的方法,随机抽取山东某三甲医院2018年上半年出院患者病历5 779份,随机抽取2018年任意1日的门诊处方200份,分析该院的抗菌药物使用强度及合理用药情况。结果该院某1工作日的抗菌药物处方占比为18.5%;门诊应用抗菌药物主要为口服用药;门诊无限制级抗菌药物使用;未发现联合应用抗菌药物的情况。该院2018年上半年的抗菌药物使用率为46.46%,使用频率略高。主要给药途径为静脉输液(72.66%),联合用药的比例较高。2018年上半年抗菌药物使用强度(AUD)前5类药物依次为氨基糖苷类、大环内酯类、硝基咪唑类、喹诺酮类和青霉素类。结论该院2018年上半年门诊抗菌药物处方的使用情况基本合理,但病房抗菌药物用药频率过高。Objective To investigate the current situation of clinical antimicrobial use in a third-class hospital in Shandong Province,and to guide the rational use of antimicrobial drugs.Methods A retrospective field survey was conducted to randomly select 5779 medical records of discharged patients from a third-class hospital in Shandong Province in the first half of 2018 and 200 prescriptions on any day in 2018.Results The proportion of antimicrobial prescriptions on a certain working day was 18.5%;the main antimicrobial drugs used in outpatient department were oral;the use of unlimited antimicrobial drugs in outpatient department;and no combined use of antimicrobial drugs was found.The use rate of antibiotics in the first half of 2018 was 46.46%,and the frequency of use was slightly higher.The main route of administration was intravenous infusion(72.66%),and the proportion of combined medication was higher.The top 5 AUD drugs in the first half of 2018 were amino glycosides,microclines,nitroimidazoles,quinolones and penicillin.Conclusion The use of antimicrobial prescriptions in the first half of 2018 in our hospital is basically reasonable,but the frequency of antimicrobial use in the ward is too high.
